摘要
目的:研究糖尿病合并心血管疾病对心率变异性(heart rate variability,HRV)的影响,探讨预测性指标。 方法:对44例合并高血压、冠心病及48例单纯糖尿病患者进行24h HRV时域分析,并进一步发现其昼夜变化规律,同时与年龄、性别相当的62例正常人HRV资料比较。 结果:①糖尿病患者各项时域分析指标均较正常组降低(P<0.05)。②与单纯糖尿病患者比较,有心血管合并症的糖尿病患者HRV参数中正 常R-R间期的标准差(standard deviation of all normal to normal RR intervals,SDNN)、每5min正常R-R间期平均值的标准差(standard deviation of all mean 5 minute RR intervals,SDANN)、每5min正常R-R间期标准差的平均值(mean of the standard deviation of au NN intervals for all 5 min segments,SDNN-index)、相邻正常R-R间期差值的均方根值(the root mean square successive differences,RMSSD)明显降低(P<0.05)。③昼夜资料分析显示糖尿病患者白天和夜间SDNN、SDNN-index、RMSSD、相邻正常R-R间期差值>50 ms的计算占总R-R间期数和百分比(percent of the number of time that the difference between adjacent normal RR intervals is greater than 50 ms in the total number of NN intervals,PNN50)均较正常组明?
AIM: To study the effect of diabetes complicated by cardiovascular diseases on heart-rate variability (HRV), and to explore the predictive index. METHODS: The time range of 24 hour HRV was analyzed in 44 patients with diabetes complicated by hypertension and coronary heart disease and 48 patients with simple diabetes. Circadian alternation regularity of the 48 patients with simple diabetes was found further. Meanwhile, the comparison was made with the HRV data of 62 healthy persons of the same age and sex. RESULTS: (1) The analysis index of each time range in patients with diabetes was lower than that in the normal group (P < 0. 05) . (2) Compared with patients with simple diabetes, standard deviation of all normal to normal RR intervals (SDNN), mean of the standard deviation of all NN inrervals for all 5 min segments (SDANN), mean of the standard deviation of all NN inrervals for all 5 min segments (SDNN-index) and the root mean square successive differences (RMSSD) were decreased obviously in diabetic patients complicated by cardiovascular disease ( P < 0. 05) . (3) The analysis of cir-cadian data showed that night-and-day SDNN, SDNN-index, RMSSD, and percent of the number of time that the difference between adjacent normal RR intervals is greater than 50 ms in the total number of NN intervals (PNN > 50) in diabetic patients were significantly lower than those in the normal control group ( P < 0. 05) . The change of measuring value in diabetic patients complicated by hypertension and coronary heart disease was more significant than( P < 0. 05).CONCLUSION: There is HRV abnormality in diabetic patients, the abnormal degree is directly proportional to the severity of patients' condition, the abnormality of circadian rhythm in patients with cardiovascular combination is more significant, which suggests that HRV analysis in diabetic patients helps to predict the occurrence of malignant cardiovascular event.
