摘要
目的 探讨急性肺栓塞的诱发因素与发病的关系 ,临床症状、体征及辅助检查在早期诊断中的价值。 方法 对我院 1999～ 2 0 0 2年收治确认的急性肺栓塞病人 10例 ,男 6例 ,女 4例 ,年龄 4 2～ 79岁 ,平均年龄 6 6 2岁 ,对其临床表现 ,诱发因素 ,辅助检查进行分析。 结果 10例病人中均有不同程度的出汗、血压下降 ;9例表现胸闷 ,呼吸困难 ;6例以晕厥为首发症状 ,猝死 6例 ;心电图 :电轴右偏 9例 ,4例呈典型“SⅠQⅡTⅢ”改变 ,4例表现为完全性右束支传导阻滞 ,3例胸前导联呈“冠状T波” ,胸部X线拍片 10例中存活 4例具备典型X线表现 ;3例行动脉血气检查具有低氧血症。病史中 6例患有慢性心肺疾病 ,3例与外科手术有关。 结论 急性肺栓塞病人大多具有诱发因素 ,临床症状 ,体征多种多样 ,部分病人以晕厥为首发症状 ,及时进行心电图。
Objective To investigate the relationship between the inducing factors and onset of acute pulmonary embolism and study the value of clinical symptom sign and clinical assessment in the early diagnosis.Methods The inducing factors、 clinical symptom and clinical assessment of 10 patients with established acute pulmonary embolism (hospitalized from 1999～2002,6 male,4 female,age 42～79,average 66.2 years old) were analyzed retrospectively.Results All 10 patients with pulmonary embolism sweated and the blood pressure reduced, 9 patients had dyspnea, 6 patients had syncope as the first clinical symptom and 6 patients had sudden death. electrocardiogram(ECG): 9 patients had the vesultand vectors turn right, 4 patients had typical findings on “S ⅠQ ⅡT Ⅲ”. 4 patients had the complete right bundle branch block, 3 patients had coronary T wave. 4 patients of 10 survived had typical findings of chest X rays. In the blood gasanalysis 3 patients had hypoxemia. In the history of illness 6 patients had chronic pulmonary heart disease, 3 patients had relevance to surgeries.Conclusion Most of the acute pulmonary embolism had the inducing factors and had kinds of clinical symptoms and signs. Some patients had syncope as the first clinical symptom. Making ECG and the chest X rays in time is important to diagnose this disease in junior hospitals.
