摘要
目的探讨颈椎双向牵引床在寰枢椎脱位中的应用价值。方法回顾性总结分析我院1984～2001年应用颈椎双向牵引技术治疗92例寰枢椎脱位患者的临床效果。结果5例新鲜脱位均能复位,87例陈旧性病例中有7例未经前路松解术而能复位;72例须经前路松解后再牵引能缓慢复位,有8例仍不能复位。结论颈椎双向牵引技术是对颈椎单向牵引的进一步革新和完善,牵引功效明显优于单向牵引,适应于经单向牵引无效的寰枢椎骨折脱位。
Objective To investigate practical value of bivector traction for atlantoaxial dislocation. Methods 75 cases of atlantoaxial fracture and dislocation were treated with bivector traction from 1984 to 2001, and their clinical effects were analyzed. Results 5 cases of fresh injury could be all reduced. Of the 70 cases of old injury, only 4 cases could be reduced without anterior release, 60 cases could be reduced after anterior decompression, and the other 6 cases could not be reduced. Conclusion With better clinical effect, the technique of bivector traction is an innovation or improvement of the traditional skull traction. It is suitable for patients with atlantoaxial dislocation which cannot be reduced with traditional skull traction.
