摘要
目的 :探讨后房型人工晶状体后脱位发生的原因及处理方法。方法 :对 6例后房型人工晶状体后脱位患者临床资料 ,进行回顾性复习 ,以发现其发生原因及最佳处理方法。其发生原因与摘出白内障术中晶状体后囊破裂或悬韧带严重损伤有关。对 6例患者中 5例 ,采用玻璃体切除及睫状沟缝线固定术 ;1例观察。结果 4例人工晶状体复位成功 ,裸眼视力 0 3～0 8,1例并发眼内炎再次取出人工晶状体 ,观察的 1例矫正视力 0 6。结论 :后房型人工晶状体后脱位与后囊膜破裂有关 ,玻璃体手术及睫状沟缝线固定可以使人工晶状体复位 ,提高视力。
Objective:To study the cause of dislocation of posterior chamber intraocular lenses(IOL)and its management.Methods:The deta of 6 patients with dislocation of posterior chamber IOL were reviewed,and found out the dislocation was related to the laceration of posterior lens capsule or ciliary zonule.Of the 6 cases with dislocation of IOL into vitreous cavity,5 cases were treated with vitrectomy and fixation of IOL at ciliary sulcus.1 case was observed only without any operation.Results:The dislocated IOL were replaced successfully in 4 cases,and the visual acuity without correction was 0 3 to 0 8.The IOL was removed in 1 case because of endophthalmitis.The corrected visual acuity in the case without surgery was 0 6.Conclusion:Dislocation of the posterior chamber intraocular lens into vitreous cavity is caused by laceration of posterior lens capsule or ciliary zonule.Vitreoretinal surgery and fixation of IOL at ciliary sulcus may replace the IOL to the correct position and improve the visual acuity.Endophthalmitis should be prevented.
