摘要
目的 探讨急性双表型白血病 (biphenotypicacuteleukaemiaBAL)的临床与生物学特点、治疗及预后。方法 本文总结报道了 5例BAL的诊治资料 ,所有患者均结合细胞形态学、组织化学染色、免疫表型 ,参照急性白血病免疫学特征欧洲协作组 (EuropeanGroupfortheImmunologicalCharacterizationofAcuteLeukemiasEGIL)评分系统诊断。结合相关文献复习。结果 全部患者均有相应的肿瘤相关的症状。 5例患者中 ,免疫表型为共同表达髓系和B淋巴系标志者 2例 ,共同表达髓系和T淋巴系标志者 2例 ,同时表达三个细胞系标志者 1例。CD3 4阳性者 2例。治疗宜采用兼顾急性髓细胞白血病 (AML)及急性淋巴细胞白血病 (ALL)的化疗方案 ,但患者对治疗反应差。结论 BAL具有独特的临床、生物学和预后特征。
Objective To investigate the clinical and biological features,treatment and prognosis of biphenotypic acute leukaemia(BAL).Methods In this study,five cases of BAL were retrospectively reviewed,combining relative literatures.This entity had been defined according to a scoring system proposed by the European Group for the Immunological Characterization of Acute Leukemias(EGIL)by the study of morphology,cytochemistry and immunophenotype.Results All patients had a tumoural syndrome with 2 cases coexpressing B lymphoid and myeloid antigens,2 cases coexpressing T lymphoid and myeloid antigens,and 1 case coexpressing B,T lymphoid and myeloid antigens.CD34 was expressed in 2 cases.These patients were treated with regimens suitable for both AML and ALL,but poor responses were demonstrated in patients with BAL.Conclusion The results of our study show that patients with BAL have unique clinicalbiological and prognostic features.
