摘要
报告1例来自非流行区的播散型荚膜组织胞浆菌病。患者男,32岁。首发症状为高热、头痛,血细胞进行性下降,血真菌培养阴性。骨髓真菌培养25℃时为霉菌相,镜下可见齿轮状大分生孢子,37℃时为酵母相。经给予两性霉素B和氟胞嘧啶治疗后痊愈。提示本病在我国有散发,且不限于某些地区和人群。避免误诊的关键是提高对本病的认识,以及对可疑病例做真菌学检查。
A case of disseminated histoplasmosis was reported.The male patient aged32with high fever and headache came from non-endemic area.The identity of the organism was confirmed by bone marrow culture.The organism appeared to be mould at25℃,yeast at37℃.When it appeared to be mould,it was found with characteristic denticle macronidia under microscope.The patient received amphotericin B in combination with5-FC for treatment and was completely cured.It is indi-cated that histoplasmosis may sporadically happen in non-endemic areas in our country.All people may be infected with histoplasmosis.The key points to avoid misdiagnosis are earlier recognition and laboratory identification of the fungus in sus-pected patients.
