摘要
目的 探讨超声引导微波介入治疗实体瘤的疗效和可行性。 方法 199例实体瘤 (肝癌 182例、肺癌 5例和骨肿瘤 12例 )进行超声引导下微波介入治疗。 结果 所有病例治疗后全身情况好转 ,症状减轻或消失。CT检查 ,肿块缩小 ,无强化。超声引导下活检显示肿瘤大片坏死。 <5cm肝癌有效率 (CR +PR) 92 5 % (74 80 ) ,无效 (NC) 7 5 % (6 80 ) ,1、2、3年存活率分别是 88 8% (71 80 ) ,72 5 % (5 8 80 ) ,6 2 5 % (5 0 80 )。 >5cm肝癌有效率 (CR +PR) 6 0 8% (6 2 10 2 ) ,NC39 2 % (4 0 10 2 ) ,1、2、3年生存率 6 7 6 % (6 9 10 2 ) ,4 9 1% (5 0 10 2 ) ,39 2 % (4 0 10 2 )。骨肿瘤和肺癌有效率 (CR +PR)分别为 10 0 % (12 12 )和 6 0 % (3 5 )。无其他严重并发症发生。 结论 超声引导微波介入治疗肝、肺和骨肿瘤是一种安全、可行、有效的方法。
Objective To evaluate the feasibility and effect of ultrasound-guided microwave intervention in the management of some solid carcinoma. Methods Ultrasound-guided microwave interventional therapy was used in 199 cases of solid carcinoma, which included 182 cases of hepatic cancer,5 cases of lung cancer and 12 cases of bone tumor. Results After treatment the general condition of all patients had a favorable change, with the symptoms lessen or free. CT scans showed a regression of tumors and no enhancement. Ultrasound-guided biopsy revealed complete necroses of tumors. In patients with hepatic cancer ≤5cm, the effective rate (CR+PR) was 92 5% (74/80), no change (NC) rate was 7 5% (6/80), and the 1-, 2-, 3-year survival rates were 88 8% (71/80), 72 5% (58/80) and 62 5% (50/80), respectively, whereas in patients with >5cm hepatic cancer, the effective rate (CR+PR) was 60 8% (62/102), no change (NC) rate 39 2% (40/102), and the 1-, 2-, 3-year survival rates 67 6% (69/102), 49 1% (50/102) and 39 2% (40/102), respectively. The effective rates (CR+PR) in patients with bone tumor and lung cancer were 100%(12/12) and 60%(3/5), respectively. There were no other severe complications found. Conclusions Ultrasound-guided microwave intervention is a safe, effective and feasible method for hepatic cancer, lung cancer and bone tumor.
