摘要
目的:研究周围神经损伤以后,神经远端与健康神经行端侧吻合,靶器官定期注射神经生长因子(NGF),对神经再生及靶器官功能恢复的影响。方法:30只雌性Wistar大白鼠,随机分为NGF组,NS组及正常组。前两组切断右下肢腓神经,远端同健康胫神经行端侧吻合,手术当天开始右小腿外侧注射NGF200U,1次/d,持续两周,对照组注射等量生理盐水(NS),分别于12周后行胫前肌湿重、电生理、再生神经、靶肌肉组织学检查。结果:胫前肌湿重NGF组为(0.80±0.14)g,高于NS组(0.59±0.12)g(T=5.60,P<0.01)术后12周NGF组复合肌肉动作电位振幅犤(3.77±0.35)mV犦,潜伏期犤(5.595±0.893)ms犦和运动神经传导速度犤(26.50±2.53)m/s犦,均优于NS组犤(2.53±0.74)mV,(8.327±0.802)ms,(19.62±1.71)m/s,T=9.91,8.04,10.43,P<0.01犦。结论:周围神经端侧吻合后靶器官内注射NGF,能促进神经侧支再生,靶器官功能恢复。
AIM:To evaluate the effect of nerve growth factors(NGF) injected periodically into target organ in promoting the functional recovery of target organ and neural regeneration when the affected nerve end was inosculated with the healthy nerve end after peripheral nerve injury. METHODS:Thirty male Wistar rats were randomized into NGF group,normal saline(NS) group and normal control group.The peroneal nerves of the right lower limbs of the rats in the two former groups were cut,and distal ends were inosculated with those of the healthy ones in an end-to-side fashion. NGF 200 μwas injected into the lateral right leg immediately after operation,once a day continuously for 2 weeks.The rats in the NS group were injected with equivalent saline.Until the twelfth week, the wet weight was measured, and electrophysiology,neurology and muscular histology were performed in peroneal muscle,regenerative nerve and muscle fiber.RESULTS:The wet weight of the tibialis anterior muscle was(0.80±0.14) g in the NGF group, obviously higher than in the NS group [(0.59±0.12) g](T=5.60,P< 0.01).At 12 weeks after operation, the NGF group had the higher amplitude of compound muscle action potential[(3.77±0.35)mV], the shorter latency [(5.595±0.893) ms] and the faster motor nerve conduction velocity [(26.50±2.53) m/s]as compared with the NS group [(2.53 ±0.74) mV,(8.327±0.802) ms,(19.62±1.71) m/s](T=9.91,8.04,10.43,P< 0.01).CONCLUSION:NGF injected into target organ can promote the regeneration of nerve branch and the functional recovery of target organ.
