摘要 目的 :用三硝基苯磺酸 (TNBS)和乙醇建立Crohn病 (CD)的二种大鼠模型 ,并比较不同乙醇用量对模型的影响。方法 :肠道灌注含TNBS 30mg的溶液 (5 %TNBS0 .6mL +无水乙醇 0 .3mL) ,建立大鼠CD模型 (称为TNBS/E1实验组 ) ,分别于 1,3,7,14 ,2 1,2 8d处死 ,观察各时点结肠大体形态和组织学改变 ,设肠道灌注生理盐水 (0 .9mL)、5 %TNBS(0 .6mL) +5 0 %乙醇 (0 .3mL)两个对照组 (分别称为盐水对照组及TNBS/E2 对照组 ) ,分别于 1、14d处死。结果 :TNBS/E1实验组大鼠致炎后第 1周主要表现为肠壁高度水肿、出血、糜烂、溃疡形成及大量中性粒细胞浸润的急性炎症 ;第 2周后有类似于铺路石样征 ,甚至节段性炎症 ,光镜发现在粘膜下出现淋巴细胞聚集 ,裂隙溃疡、肉芽肿形成 ;2 1d后溃疡开始愈后 ,炎症慢性化 ,大体形态开始恢复正常 ,但光镜下仍有明显炎症反应。TNBS/E2 对照组大鼠的个体差异很大 ,在第 14天时大部分肠壁形态基本恢复正常 ,光镜下炎症表现差别较大。结论 :5 %TNBS0 .6mL +无水乙醇 0 .3mL灌肠造成的模型与人类CD近似 ,炎症持续时间较长 ,是一种研究CD发病机制和评估药物疗效的工具。 Objective:To improve the method of establishing a rat chronic colonitis model similar to human Crohn's disease(CD)by the intraluminal instillation of a solution containing 2,4,6-trinitrobenzenesulfonic acid(TNBS)and ethanol.Methods:The rat Crohn's disease model were made with 0.6 mL of 5% TNBS plus 0.3 mL of anhydrous alcohol(TNBS/E 1 group),0.6 mL of 5% TNBS plus 0.3 mL of 50% alcohol(TNBS/E 2 group),and 0.9mL of normal saline(normal saline group),respectively,and then the rats were killed at various time points.Colonic inflammation and damage were assessed macroscopically and histologically.Results:In TNBS/E 1 group,acute colonic inflammation including edema,hemorrhage,erosion,ulceration and the predominant infiltration of neutrophils occurred during the 1st week;pebble's sign and even segmental inflammation appeared following the 2nd week,and the histological examination found that there were lymphocyte infiltration,ulceration and granulation;there was a normal appearance of the damaged colons after the 3rd week,but the chronic inflammation was easily observed.The colonic inflammation responses were greatly different in TNBS/E 2 group.The most of the damaged colons were generally normal at the 2nd week,but there was an obvious individual difference in histological findings.Conclusion:This rat model simulates human CD,which could be used to study the pathogenesis and to evaluate the pharmaceutical effect of CD.
