摘要
目的 探讨毁损性手外伤治疗方法。方法 用废弃组织转位、联合组织瓣移植治疗毁损性手外伤 19例 ,一级组合 (两个组织瓣参与移植 ) 8例 ,二级组合 (三个组织瓣参与移植 ) 3例 ;复合组织移植 6例。异位再植 3例。参与移植组织种类包括皮瓣 11块 ,皮瓣范围最大 30cm× 10cm ,最小 9cm× 8cm ;足趾 13组 2 4趾 ,游离脚指甲瓣 2组 5个。结果 19例 4 0个参与移植的组织 38个成活 ,其中 1例 2个移植组织坏死 ,成活病例创面均Ⅰ期愈合。本组 19例中 16例经 6个月 - 9年的随访 ,患者手部均恢复夹持、抓握等基本功能。移植的组织均有良好的感觉 ,再造指两点辨别觉 3.5 - 14mm ,皮瓣均恢复保护性感觉。结论 根据伤情合理地采用废弃组织转位、联合组织瓣移植等显微外科技术 。
Objective To investigate methods in treating hand destructive injury.Methods Useless,composited tissue flaps were done to treat hand destructive injuries in 19 cases,in which there were 8 cases of one band composition(two sort tissue),in which there were 3 cases of two band composition(there sort tissue),6 cases were composite tissue flap and 3 cases were pollicisative replantation.Trans far tissue including:flapscll,the size of the flap ranged form 30cm×10cm to 9cm×8cm,toe 13 groups(24 toes),free toe-nail flap 2 group(5).Results 38 transferred tissues were survived completely in 40(19 cases),2 transferred tissues(1 cases) died,lived cases were satisfactory.16 cases of the 19 cases were evaluated as satisfactory after 6 months to 9 years follow up.The function(hold grasp etc)completely restored,the transferred tissues felt completely,reconstructed finger's function of the two point measurement was 3.5-14mm,Protetive feel were restored of the flaps.Conclusion According to degree of injury,microsurgery such as useless tissue flap,composite tissue flap etc which to reconstruct the destructive hand primary function is a well treatment of choice.
