摘要
目的 通过检测糖尿病合并肺结核患者人类组织相容性Ⅱ类 (HLA II)抗原信使核糖核酸 (mRNA)的表达 ,从基因水平探测 2型糖尿病合并肺结核患者易发肺结核的原因。方法 经支气管肺泡灌洗液收集肺泡巨噬细胞 ,利用逆转录多聚酶链反应 (RT PCR)检测 4组人肺泡巨噬细胞上的人白细胞DR抗原 (HLA DR)和人白细胞DQ抗原(HLA DQ)mRNA的表达。结果 2型糖尿病合并肺结核组 (A组 )、2型糖尿病组 (B组 )巨噬细胞上这两种抗原mRNA相对含量的表达较肺结核组 (C组 )及正常对照组 (D组 )的表达明显减少 (P <0 .0 5 ) ;而A、B组间以及C、D组间巨噬细胞上这两种抗原mRNA的表达 ,差异无统计学意义 (P >0 .0 5 )。结论 糖尿病患者肺泡巨噬细胞HLA II类抗原mRNA的表达降低 ,导致巨噬细胞HLA II类抗原的表达减少 ,从而导致巨噬细胞识别和提呈能力的下降是引起糖尿病患者好发肺结核的原因之一。
Objective To study the pulmonary immunity in the case when diabetes mellitus complicated with pulmonary tuberculosis by detecting mRNA expression of human leucocyte antigen(HLA-II) antigens on alveolar macrophages(AM) and to understand the reason of the patients with diabetes mellitus who predisposed pulmonary tuberculosis.Methods Through the fibric brochus-endoscopy examination, AM sample was obtained from patients with diabetes mellitus complicated with pulmonary tuberculosis (group A), diabetes (group B), pulmonary tuberculosis (group C), and controls (group D) and the mRNA expression of HLA-DQ and HLA-DR on AM was detected by reverse transcriptase-polymerase chain reaction.Results The expression of HLA-II antigens mRNA on AM in group A and group B was lower than that in group C and D (P< 0.05); There was no significant difference between group A and B ,or C and D (P> 0.05).Conclusion The less expression of HLA-II antigens mRNA on AM in patients with diabetes may be the one reason of patients with diabetes mellitus who predisposed pulmonary tuberculosis.
