电刺激小脑顶核对孔源性视网膜脱离复位术后视功能恢复的影响 被引量:3

摘要 目的 观察并评估电刺激小脑顶核对孔源性视网膜脱离 (rhegmatogenousretinaldetachment,RRD)患者复位术后视功能恢复的影响。方法 将符合入选标准的 4 6例 4 6眼孔源性视网膜脱离患者随机分为 2组。复位术后给予常规药物治疗的患者作为对照组 ,治疗组则是复位术前及术后在常规药物治疗的基础上加用脑循环功能治疗仪进行电刺激小脑顶核。手术前后均以视力和采用Tubinger自动视野分析仪 0°~ 80°范围的快速阈值程序测定的视网膜光敏感度值作为指标来判断视网膜功能 ,并比较 2组患者RD复位术后视功能的改善情况。结果 对照组术后视力进步 4例 ,不变 19例。治疗组术后视力进步 6例 ,不变 17例。 2组比较术后视力的改变无统计学意义 (P >0 .0 5 )。 2组视网膜脱离解剖复位术后全视野的平均光敏感度明显提高 ,平均缺失敏感度明显下降。治疗组与对照组比较 ,术后平均光敏感度的提高呈非常显著性差异 (P <0 .0 1) ,术后平均缺失敏感度的下降亦呈显著性变化 (P <0 .0 5 )。结论 电刺激小脑顶核对视网膜脱离解剖复位术后视网膜功能的恢复具有改善作用 ,这一新的治疗途径可在临床上应用。 Objective To observe and analyze the effects of electrical stimulation on cerebellar fastigial nucleus on the visual function recovery of the patients after retinal reattachment surgery.Methods Forty six cases (46 eyes) with rhegmatogenous retinal detachment were divided into two groups randomly.The postoperative patients in the control group were treated with routine drug.The patients in the therapeutic group were treated with routine drug plus electrical stimulating cerebellar fastigial nucleus before and after operation.Under the fast threshold program within 80°,we used Tubinger Field Analyzer to measure the retinal light sensitivity (RLS).The RLS and the visual acuity were tested before and after operation to evaluate the retinal function and to compare the changes of visual function after retinal detachment surgery in two groups.Results In the control group the visual acuity of 4 cases was improved,that of 19 cases unchanged.In the therapeutic group,the visual acuity of 6 cases was improved,that of 17 cases unchanged.The changes of the visual acuity were no statistical differences in two groups ( P >0.05). The mean sensitivity(MS) of postoperative eyes in two groups showed a significant increase and mean defect(MD) decrease compared with preoperative eyes. About the MS and MD,there was an obvious difference between the control group and the therapeutic group after retinal reduction.Conclusion The electrical stimulation to cerebellar fastigial nucleus can help promoting the visual function of the patients after retinal reattachment surgery.The new treatment can be used in clinic.
