摘要
目的 探讨高血压患者外周血细胞形态的改变。方法 对 16 0例高血压患者和 175 1例血压正常人的血细胞形态和血清生化检查资料进行对比分析。结果 高血压组外周血白细胞计数、红细胞计数、血红蛋白、HCT、MCHC、RDW -SD和PDW -SD均高于对照组 (P均 <0 0 5 ) ,MPV则低于对照组 (P <0 0 5 )。SBP和DBP水平与年龄、体重、血清甘油三酯、肌酐和单核细胞 (% )存在一定程度的相关 (r =0 .0 71～ 0 398,P均 <0 0 5 )。SBP还与血糖、外周血白细胞计数、血红蛋白、RDW -SD和PDW -SD相关 (r =0 .0 92～ 0 2 2 2 ,P均 <0 0 1)。高血压组血清谷丙转氨酶、甘油三酯、血糖和肌酐的水平也高于对照组 (P 均 <0 0 5 ) ;而胆固醇和尿素水平与对照组相近 (P 均 >0 0 5 )。结论 高血压患者存在外周血细胞形态学的改变 ,这可能与高血压急慢性并发症的发生有一定的联系。
Objective To investigate the form changes of peripheral blood cells in the primary hypertension.Methods The blood cell form and serum chemical analysis data in 160 cases with primary hypertension(HP group)and 1 751 medical workers with normal blood pressure (control group)were collected and analyzed.Results The white blood cell count,red blood cell count,hemoglobin, hematocrit,mean cell hemoglobin concentration, red cell distribution width and platelet distribution width in the HP group were higher than those in the control group (all P<0.05); and the mean platelet volume in the HP group was lower than that in the control group.Both systolic blood pressure(SBP)and diastolic blood pressure (DBP)levels correlated highly with the age,body weight,serum triglyceride,serum creatine and monocyte cell percentage(r=0.071～0.398,all P<0.05).The SBP level also correlated well with the blood sugar,white blood cell count,hemoglobin,red cell distribution width and platelet distribution width(r=0.092～0.222,all P<0.01).The serum GPT level,triglyceride,blood sugar and creatine in HP group were also higher than those in the control group,and the serum total cholesterol and blood urea nitrogen had no significant difference between two groups(all P>0.05).Conclusion The changes of peripheral blood cell form are present in the patients with primary hypertension.That may be related to the onset of acute and chronic complications in the hypertension progress.
