摘要
目的 :提高垂体脓肿的术前CT、MRI诊断水平。材料和方法 :回顾性分析 7例垂体脓肿患者的病历资料 ,结合文献对其影像学特征 ,术前误诊及诊断情况进行分析讨论。结果 :7例患者术前诊断垂体脓肿只有 1例 ,诊断为垂体炎 1例。表现各种垂体功能低下 5例。中枢性尿崩 5例。垂体脓肿的CT表现与脑脓肿无区别。垂体脓肿MRI表现趋向T2高信号 ,T1等或低信号 ,伴有环状增强。部分患者MRI表现为T1高信号T2高信号。结论 :垂体脓肿的影像学诊断有赖于综合分析CT、MRI影像特征及脓肿的间接影像表现。垂体功能的减退及中枢性尿崩的出现对垂体脓肿的鉴别诊断有重要意义。
Purpose:To improve accuracy of image diagnosis of the pituitary abscess before operation. Materials and Methods:Seven patients with the pituitary abscess were reviewed. The image features, preoperative diagnosis were analyzed and discussed retrospectively. Results:Of seven patient,only one was preoperatively diagnosed as the pituitary abscess and one patient as the hypophysitis. In this group, the various hypopituitarism was 71.4% and central diabetes insipidus was 71.4% in clinical study. CT feature of the pituitary abscess was not different with the abscess in brain. MR image showed high signal on T2WI and isointense or hypointense signal on T1WI in the pituitary abscess. Postcontrast T1WI showed a thin rim of enhancement?Both T1WI and T2WI were high signal in part of the patients. Conclusion:CT combined with MRI could improve the accuracy of diagnosis of pituitary abscess. The clinical data of central diabetes insipidus and hypopituitarism can play a very important role in differential diagnosis of pituitary abscess.
