摘要
目的 :分析颅内原发恶性淋巴瘤的MRI影像学特征。材料和方法 :对 2 5例经手术及组织病理证实的颅内恶性淋巴瘤的MRI表现进行回顾性分析。结果 :2 5例中单发病灶 18例 ,多发病灶 6例 ,单纯室管膜下及蛛网膜下腔弥漫性浸润 1例。平扫检查 2 5例在T1WI呈稍低或低信号 ,T2WI呈等信号或稍高信号。增强扫描 2 0例中 17例病灶呈明显团块状增强 ,2例病灶轻度增强 ,室管膜下及蛛网膜下腔弥漫性浸润 3例 (单纯室管膜下及蛛网膜下腔弥漫性浸润 1例 )呈线样及小结节样增强 ,增强程度较均一。结论 :颅内原发恶性淋巴瘤的MR信号强度及增强方式对诊断和鉴别诊断具有重要价值。
Purpose: To analyze MRI features of primary intracranial malignant lymphoma. Materials and Methods:MRI appearences of 25 cases with primary intracranial malignant lymphoma which were proved by surgery and histopathology were retrospectively analyzed. Results:Solitary lesion was found in 18 cases, and multiple lesions in 6 cases, In the other 1 case diffuse subependymal and subarachnoidal infiltration was the only finding. 25 cases underwent plain MR scan demonstrated slight hypointensity or hypointensity on T1WI and isointensity or slight hyper intensity on T2WI. Among the 20 cases underwent enhanced MR scan, 17 cases were stongly massly enhanced, 2 cases were slightly enhanced, 3 cases with diffuse subependymal and subarachnoidal infiltration (including 1 case in which subependymal and subarachnoidal infiltration was the only finding) demonstrated linear and small nodular enhancement. Conclusion:The signal intensity and enhancing pattern of primary intracranial malignant lymphoma on MR images are of great value in the diagnosis and differential diagnosis.
