摘要
目的 观察稳定性心绞痛患者冠状动脉造影时血浆内皮素 - 1水平变化及其临床意义。方法 根据冠状动脉造影结果将 6 4例患者分为两组 ,冠心病稳定性心绞痛组 (SA组 )与冠脉正常组 (正常对照组 )。冠状动脉造影前及冠状动脉造影后即刻抽取血液标本测定血浆内皮素 - 1水平并记录动脉收缩压与舒张压。结果 两组冠状动脉造影所耗时间无明显差异 (P >0 0 5 ) ,冠状动脉造影前及冠状动脉造影后即刻平均动脉压在SA组分别为 137/ 86mmHg和 14 1/ 90mmHg(P <0 0 5 ) ,正常对照组分别为 136 / 85mmHg和 138/ 87mmHg(P >0 0 5 ) ;冠状动脉造影前及冠状动脉造影后即刻血浆内皮素 - 1水平在SA组分别为 5 5 1± 1 2 8ng/L和 6 94± 1 37ng/L(P <0 0 1) ,正常对照组分别为 5 10± 1 13ng/L和 5 14± 1 16ng/L(P <0 0 5 )。 结论 冠状动脉造影可激发冠心病稳定性心绞痛患者的内皮细胞反应 ,导致血浆内皮素 - 1水平升高。
Objective Release of endothlin 1 (ET 1) by endothelial cells is influenced by many stimuli including hypoxia and shear stress. We hypothesized that some of these stimuli might arise during coronary angiography (CAG). The aim of this study was to explore endothelial response in patients with chronic stable angina undergoing CAG by evaluating plasma ET 1. Sixty four patients with a mean age of 61 years (42 to 74 years) were included in the study, and subdivided into two groups (stable angina, SA group and normal control) according to the results of CAG. The mean time of the CAG procedure was 22.4 minutes (12~35 minutes). Immediately before and after CAG blood sampling, systolic and diastolic blood pressures were measured. The mean blood pressures before and after CAG were 137/86 and 141/90 mmHg in SA group ( n =38), and 136/85 and 138/87 mmHg in normal control ( n =26) , respectively. Significant increased level of plasma ET 1 was observed in SA group after CAG (5 51±1 28 ng/L versus 6 94±1 37 ng/L) compared with normal control (5 10±1 13 ng/L versus 5 14±1 16 ng/L). Our study indicated that increased ET 1 production appears to be a response of endothelial function to mechanical stimulus during CAG procedure in patients with SA.
