摘要
目的:研究认知功能训练对患有结构性失用的脑卒中患者平衡功能的影响。方法:1997-01/2001-12期间患有结构性失用的偏瘫患者59例,利用抽签法随机分为两组,观察组30例,对照组29例。两组患者均接受常规的、综合的物理治疗(包括物理因子治疗和肢体功能、翻身、体位转移和坐位和站位平衡功能训练),及作业治疗(包括跨越中线的作业活动和日常生活活动指导)。观察组除了上述治疗外,还进行视-空间知觉认知训练(包括几何图形临摹、火柴拼图、拼搭积木等训练)。疗程为6周。治疗前后,两组患者的下肢运动功能用简式Fugl-Meyer进行评估、平衡功能则用Fugl-Meyer平衡量表、结构性失用用几何图形临摹法进行评估,结果分4等级。结果:治疗前两组患者在年龄、性别、疾病分型、患侧、病程、Fugal-Meyer运动量表下肢积分、Fugal-Meyer平衡量表及几何图形临摹法评定方面均无统计学意义。6周后两组患者下肢Fugl-meyer积分差异没有显著性意义(8.47±2.17比7.62±2.05,t=1.27,P=0.48),而在几何图形临摹法评定(3.13±0.78比1.90±0.82,t=8.47,P=0.00)、Fugl-Meyer平衡量表方面(7.17±2.07比3.52±1.33,t=8.93,P=0.00)观察组优于对照组。结论:空间认知训练对偏瘫患者的平衡功能有促进作用。
AIM:To investigate the effect of cognitive training on equilibrium function of stroke patients with constructional apraxia. METHODS:Fifty nine stroke patients with constructional apraxia (1997 01 2001 12) were randomly divided into the observation group(n=30) and control group(n=29).Both groups received routine comprehensive physiotherapy(including physical agent therapy, lamb function, turn over, posture shifting, sitting and standing equilibrium function training) and performance therapy(including performance of striding over the midline and instruction of activities of daily living(ADL), besides, the observation group received the cognitive training of visual space perception (including geometric figures facsimile, match picture puzzle, brick building training and so on).The course was 6 weeks. Before and after the treatment, the lower lamb motor function was evaluated with simple Fugl Meyer, equilibrium with Fugl Meyer balance scale, and constructional apraxia with geometric figures facsimile test. And the result was classified into 4 levels. RESULTS:There were no significant differences of the two groups in age, sex, type of disease, diseased side, score of lower lamb in Fugal Meyer scale, Fugal Meyer balance scale and geometric figures facsimile before treatment. After 6 weeks, there were no significant differences between the two groups in Fugl Meyer score of lower lamb(8.47±2.17 and 7.62±2.05,t=1.27,P=0.48), while the evaluation of geometric figures facsimile (3.13±0.78 and 1.90±0.82,t=8.47,P=0.00),Fugl Meyer balance scale(7.17±2.07 and 3.52±1.33,t=8.93,P=0.00)in observation group were better than those in the control group. CONCLUSION:Space cognitive training can facilitate the recovery of the equilibrium function in hemiplegic patients.
