摘要
目的 研究P2 7kip1在胰腺癌发生发展中的作用及预后意义。方法 用免疫组织化学方法研究P2 7kip1在 39例胰腺癌手术切除标本及 5例正常胰腺标本中的表达 ,其结果与临床病理资料及CyclinD1的表达进行相关性分析。结果 39例胰腺癌中 ,2 6例 (6 6 7% )P2 7kip1呈低水平表达 (阳性细胞数 <5 0 % ) ,仅 13例 (33 3% )表达水平与正常胰腺组织相似 (阳性细胞数 >5 0 % )。相关分析显示 :P2 7kip1表达缺失或下调与肿瘤的不良分化相关 (P =0 0 3) ,但与性别、年龄及TNM分期无关。Kaplan Meier生存曲线表明 ,肿瘤呈高表达的病人术后总体生存期较低表达者明显延长 (P <0 0 1)。此外 ,还发现P2 7kip1与CyclinD1的表达呈负相关趋势 ,但未达到显著性水平 (P =0 0 98)。 结论 P2 7kip1在胰腺癌发生、发展中有重要的作用 ,可作为胰腺癌切除后一种新的预后指标。
Objective To study the role of P27 kip1 in pancreatic carcinogenesis and progression. Methods The expression of P27 kip1 was determined in pancreatic samples from 39 patients with pancreatic carcinoma and 5 healthy individuals with immunohistochemistry. The results were correlatively analyzed with clinicopathological data and Cyclin D1 overexpression. Results For the 39 patients with pancreatic carcinoma, low (<50% of positive cells) and high (>50% of reacting cells) P27 kip1 expression was detected in 26 (66.7%) and 13 (33.3%), respectively. Low P27 kip1 level was associated with higher histological grade ( P =0.03) but not with other clinicopathological parameters including TNM status, sex and age. Furthermore, Kaplan-Meier curves of survival showed that tumors with low P27 kip1 level were associated with poorer overall survival than those with high P27 kip1 level ( P <0.01). In addition, negative correlation between P27 kip1 and Cyclin D1 expression was observed. However, this did not reach a significant level ( P =0.098). Conclusions P27 kip1 plays an important role in occurrence and development of pancreatic carcinoma and can be used as an index predicting the prognosis of the tumor.
