摘要 背景与目的:第10号染色体同源丢失性磷酸酶-张力蛋白基因(phosphataseandtensionhomologydeletedonchromosometen,PTEN)是迄今发现的第一个具有磷脂酶活性的抑癌基因,该基因突变或缺失及蛋白表达异常与多种肿瘤的发生、发展密切相关。本研究旨在探讨抑癌基因PTEN与子宫内膜癌发生、发展的关系。方法:应用半定量逆转录聚合酶链反应(reversetranscriptionpolymerasechainreaction,RT-PCR)方法测定24例子宫内膜癌组织、10例子宫内膜非典型增生组织、10例子宫内膜增生组织和10例正常子宫内膜组织的PTENmRNA表达水平;SP免疫组化法测定73例子宫内膜癌组织(其中腺癌57例)、25例子宫内膜非典型增生组织、71例子宫内膜增生组织和31例正常子宫内膜组织的PTEN蛋白表达水平,并将结果与各病例的组织学类型、组织学分级、肌层浸润深度和临床分期等生物学行为进行相关性分析。结果:在转录水平和蛋白水平,子宫内膜腺癌组和癌前病变组中PTEN的表达均明显低于内膜增生症组和正常子宫内膜组,各组组织中PTENmRNA相对含量分别为0.35±0.13、0.46±0.11、2.32±0.32、2.45±0.51;PTEN蛋白表达缺失率分别为66.67%(38/57)、76.00%(19/25)、5.63%(4/71)和0%(0/31)。PTEN蛋白表达缺失率与其组织学类型和组织学分级有关(P均<0.01),? BACKGROUND &OBJECTIVE:A new candidate tumor suppressor g ene,called phosphatas e and tension homolog y deleted on chromosome ten(PTEN),was the first g ene that was found to b e phospholipase tumor suppressor g ene.Loss of PTEN f unction by mutation or other mechani sms is closely related to tumorig enesis and prog ression of multiple carcino mas.This study was desig ned to investig ate th e expression of PTEN in carcinog enes is and development of endometrium carcino ma.METHODS :The expression of PTEN were detected by reverse transcript ion polymerase chain reaction(RT-PCR)for 24cases with endometrial carcinoma ,10cases with endometrial atypical hyperplasia,10cases with endometr ial hyperplasia,and 10cases with normal endometrium and by SP immunohistochemical methods for 73cases with endometrial carcinoma,25cases wit h endometrial atypical hyperplasia,71cases with endometrial hyperplasia,and 31cases with normal endometriu m.The results were compared with clinical parameters(histolog ical classification,differentiation,depth of myometrium invasion,and clinical stag e ).RESTULTS :PTEN expression levels of both RNA and protein in patients with endometrial carcinoma and endometrial atypical hyperplasia were sig nificantly low er than those in patients with endometrial hyperplasia and normal endometrium.mRNA relative values were 0.35±0.13,0.46±0.11,2.32±0.32,and 2.45±0.51,respectively.Loss of PTEN expressi on rates were 66.67%(38/57),76.00%(19/25),5.63%(4/71),and 0(0/31),respectively.Loss of PTEN express ion in patients with endometrial carcin oma was sig nificantly related to his tolog ical classification(P<0.0001)and differentiation(P=0.0349).It was not related to depth of myometrium invasion and c linical stag e (P>0.05).CONCLUSION:Loss of PTEN expression is an early event in endometrial tumorig enesis.Detection of PTEN protein may be a dia g nostic biomarker for the earliest endometrial precancers and adenocarcinoma.
