降钙素基因甲基化测定用于髓系白血病疗效和病情演变的监测

摘要 背景与目的:降钙素基因在正常细胞中常呈现低甲基化状态,其高甲基化往往意味着细胞的恶性变;在血液系统恶性肿瘤中,降钙素基因高甲基化可能与病情进展有关,但有关降钙素基因高甲基化在髓系白血病诊疗中价值的研究报道尚少。本文旨在探讨降钙素基因甲基化分析在检测急性髓系白血病(acutemyeloidleukemia,AML)微小残留病灶(minimalresidualdisease,MRD)及监测慢性髓系白血病(chronicmyeloidleukemia,CML)患者病情演变中的应用价值。方法:用内切酶HpaⅡ酶切和聚合酶链式反应(polymerasechainreaction,PCR)技术研究31例初诊期AML患者和45例CML患者(其中慢性期33例、加速期2例、急变期10例)的降钙素基因甲基化状态与病情的关系,以14名健康成人作为阴性对照组。结果:初诊期AML患者中80.6%(25/31)呈现降钙素基因高甲基化(P0.01);动态观察的5例AML患者中,4例于初诊期及完全缓解期均呈现降钙素基因高甲基化,分别于完全缓解后3、5、614个月病情复发,另1例无降钙素基因高甲基化者完全缓解后25个月病情未复发。45例CML患者中,3例慢性期患者、2例加速期患者、8例急变期患者呈现降钙素基因高甲基化,急变期组(包括加速期)甲基化率明显高于慢性期组(P0.01),3例在慢性期呈现降钙素基因高甲基化者分别于检测后10、12。 BACKGROUND &OBJECTIVE:Calcitonin g ene mostly appears hypomethylation in normal cells.Hy permethylation of calcitonin g ene f requently implies malig nancy transformation and is probably related to the prog re ss of hematolog ical malig nancy tumors.H owever,studies on the sig nificance of hypermethylation of calcitonin g en e in the diag nosis and treatment of myelog enous leukemia were rarely re ported.This study was desig ned to investig ate the value of analyzing h ypermethylation of the calcitonin g ene for detecting minimal residual disease(MRD)and monitoring disease evolution in acute myeloid leukemia(AML )and chronic myeloid leukemia(CML ).METHODS :Dig estion of DNA with Hpa Ⅱin combination with polymerase chai n reaction(PCR)was used to examine the methylation p atterns of the calcitonin g ene in 31cases with AML,45cases with CML (including 33patients in chronic phase,2in accelerated phase,and 10in blast cr isis),and 14healthy adults(used as control ).RESULTS :The hypermethylation of calcitonin g ene was found in 25of 31cases with AML (80.6%),0of 14healthy adults(0%)(P<0.01).Follow-up study of 5cases with AML showed that 4cases wh o appeared hypermethylation of calcitonin g ene at both diag nosis an d complete remission relapsed 3,5,6,and14months respectively after comple te remission;the other case without hypermethylation of calcitonin g ene did not relapse 25months after comp lete remission.Among 45cases with CML,3of 33in chronic phase,2of 2in accelerated phase and 8of 10in blast crisis showed hypermethylation of calcitonin g ene;there were sig nificant differences between chronic phase g roup and blast crisis g roup(including accelerated phase)(P<0.01);three cases with CML in chronic phase developed i nto blast crisis 10,12,and 13months after hypermethylation of calcitonin g ene were detected.CONCLUSION:Analyzing hypermethylation of calcitonin g en e may be useful in assessing the prog nosis of AML and monitoring disease evolutio n in CML.
