原发性高血压患者心率与胰岛素抵抗水平的相关性研究 被引量:10

Association of heart rate with insulin resistance in essential hypertension patients

摘要 目的 :研究原发性高血压 (EH)患者心率 (HR)与胰岛素抵抗 (IR)、血脂代谢、血压水平的关系。方法 :测定 2 0 5例EH患者 (男 85例 ,女 12 0例 )的HR、收缩压、舒张压 (DBP)、体重指数 (BMI)、血糖、胰岛素、总胆固醇、三酰甘油 (TG)、高密度脂蛋白胆固醇 (HDL C)、低密度脂蛋白胆固醇 ,稳态模式评估法计算IR指数(HOMA IR)。分析HR与其他各项参数的相关性。结果 :以HOMA IR 5 0 %位点作为判断IR的切割点 ,将EH患者分为IR组与胰岛素敏感 (IS)组 ,IR组HR[(84 .5± 11.9)次 /min]显著高于IS组 [(76 .8± 10 .8)次 /min]。HR与HOMA IR、DBP、TG呈显著正相关 ,与HDL C呈显著负相关 ;逐步回归分析显示 ,HOMA IR与TG为HR的独立预测因素 ;Logistic回归分析显示 ,HR与IR关系显著。 结论 :与IS的EH患者相比 ,伴IR的EH患者有较快的HR ;EH患者HR与IR、血脂代谢直接相关 ,独立于年龄、血压、BMI。 Objective: To study the relationship between heart rate (HR) and insulin resistance, lipid metabolism, blood pressure in essential hypertension patients. Methods: HR, systolic pressure, diastolic pressure (DBP), fasting plasma glucose, serum insulin, body mass index (BMI), total cholesterol, triglycerides (TG), high density lipoprotein-cholesterol (HDL-C), low density lipoprotein-cholesterol were determined in 205 patients with essential hypertension [85 males, 120 females]. Homeostasis model assessment was applied to assess the status of insulin resistance (HOMA-IR). The correlation between HR and other parameters were analyzed. Results: Based on the HOMA-IR, the essential hypertension patients were divided into two halves, considering as insulin-sensitive individuals (IS) those with less HOMA-IR, and insulin resistant subjects (IR) those with more HOMA-IR. HR was significantly higher in IR group( 84.5± 11.9 beat/min)than in IS group ( 76.8± 10.8 beat/min)(P< 0.01). Pearson analysis showed that HR was correlated with HOMA-IR (r= 0.233,P< 0.01), DBP (r= 0.151,P< 0.05), TG (r= 0.245,P< 0.01) and HDL-C (r=- 0.169,P< 0.05). A stepwise multiple linear regression analysis showed that HOMA-IR and TG were significant independent predictors of HR. Logistic regression analyses revealed that HR was significantly related with insulin resistant. Conclusion: In patients with essential hypertension, HR is faster in those patients with insulin resistance than that in those patients with insulin sensitivity. HR is directly related to insulin resistance and lipid metabolism. This relationship is independent of age, body mass index, blood pressure level.
