经眶缘上外侧锁孔入路治疗大脑中动脉动脉瘤:手术技术与预后 被引量:1

Superolateral orbital rim keyhole approach for middle cerebral artery aneurysms:surgical technique and outcome

摘要 目的介绍一种新的手术技术:经眶缘上外侧锁孔入路微创手术治疗大脑中动脉动脉瘤及其预后。方法对1999年10月~2001年12月经此入路手术治疗的12例大脑中动脉动脉瘤的病人进行了回顾性分析。结果所有动脉瘤均成功夹闭,无死亡病例,并发症少,创伤小,美容效果理想。病人出院时和6个月后随访,改良Rankin评分所有病人均为0级。结论采用眶缘上外侧锁孔入路治疗大脑中动脉动脉瘤易操作,切口小,开颅小,暴露充分,术时短,无需输血,是一种理想的手术治疗方法。 Objective To describe a new technique: superolateral orbital rim keyh ole approach-minimally invasive surgery and the outcome for the treatment of MCA aneurysms. Methods A retrospective review of 12 patients with MCA aneurysms ru ptured or unruptured who were operated by superolateral orbital rim keyhole appr oach was performed between October 1999 and December 2001. Results All the aneu rysms were clipped successfully. The modified Rankin scores at discharge and 6 m onths afterwards revealed 0 grade in all assessed patients. Conclusion The super olateral orbital rim craniotomy is a newly developed procedure for neurosurgeon to treat middle cerebral artery aneurysms. Although this technique is not widely used, it is likely that this minimally invasive approach will become a promisin g operative technique in the future time.
