摘要
目的探讨下消化道出血的选择性造影表现及介入治疗的疗效。方法1996年2月～2002年2月间,对42例下消化道出血患者行选择性动脉造影,并对其中15例诊断明确患者行灌注或栓塞治疗。结果明确出血部位及病变者30例,诊断准确率71.4%(30/42),30例中,小肠出血10例,大肠20例;对其中3例肠道血管畸形患者行明胶海绵栓塞治疗,成功率100%;12例行动脉内血管加压素灌注,8例出血停止,4例无效,有效率66.7%。结论选择性血管造影是诊断下消化道出血最有效可靠的检查手段,动脉内介入治疗是一种有效的止血方法。
Objective To investigate the findings and the efficacy of selective arteriography in the interventional therapy of the lower gastrointestinal tract hemorrhage(LGTH).Methods From Dec 1996 to Feb 2002,Selective arteriography was performed in 42 patients with suspected LGTH.15 patients with definite LGTH underwent the intra- arterial infusions or therapeutic embolization.Results Among all patients,definite bleeding locations and lesions seen in LGTH were found in 30 cases, its accurate was 71.4 % ,of them ,small- bowel hemorrhage was found in 10 cases and large bowel hemorrhage was found in 20 cases,of them,3 cases with interstinal tract defornmity underwent therapeutic embolization of gelatin spone,its efficacy was 100 % .Among 12 patients who underwent intra- artery infusion of the vasopressor,8 cases were non recurrence of hemorrhage while other 4 cases were non- effective so theat the efficacy was 66.7 % .Conclusion The selective arteriography is the most effective and reliable menns for diagnosing LGTH,whereas the intra- artery interventional therapy is an effective hemostasis.
