摘要
目的探讨门静脉海绵样变性(CTPV)的CT表现,以提高对本病的认识。方法回顾性分析经数字减影血管造影、手术或US证实的CTPV25例。25例均行CT平扫及双期增强扫描。结果CTPV螺旋CT表现:a)门静脉狭窄和阻塞;b)门脉走行区结构紊乱,正常结构消失,出现迂曲扩张血管样结构,门静脉期明显强化;c)在动脉期可见周围肝实质短暂灌注异常,表现为局部或带状高密度。结论CTPV螺旋CT双期扫描具有一定的特征性表现,正确诊断有赖于结合临床资料和影像学征象的综合分析。
Objective To study the CT features of cavenous transformation of portal vein(CTPV).Methods 25 patients with (male 22,female 3,age ranged from 34 to 73 years)confirmed by DSA,operation or US were retrospectively reviewed.All patients underwent abdominal precontrast and dual phases contrast.Results The main spiral CT appearances were:(1)narrow and obstructed of portal vein;(2)loss of normal structure of the portal system and presence of multiple,sinuous,periportal vascular structures which were enhanced during the portal phase;(3)transient abnormal perfusion as a homogeneous band in hepatic peripheral parenchyma in arterial phase and disappeared in venous phase.Conclusion CTPV displays relatively typical appearances on spiral CT dual phases scanning.Definitive diagnosis of CTPV depends on combination of clinical and imaging data.
