摘要
目的 :探讨甲状腺微小癌 (TMC)的彩色多普勒超声 (CDU )诊断的价值及误诊分析 ,如何提高对 TMC的早期诊断水平。方法 :分析 1990～ 2 0 0 2年经手术和病理证实的 TMC90例的 CDU诊断及临床资料。结果 :CDU诊断与病理诊断符合率为 5 7.78% (5 2 / 90 ) ,病变性质待定 12例 ,误诊为良性病变 16例。 B超引导下行细针穿刺抽吸细胞学检查 (FNAC)阳性率为 84 .2 1% (16 / 19)。结论 :虽然 CDU对 TMC的诊断有其局限性 ,但对 TMC病灶的声像图进行综合分析 ,选择恰当的检查手段 (FNAC)亦可提高 TMC的早期诊断水平。
Objective:To investigate the clinical significance of color doppler ultrasonography (CDU) in the dianosis of thyroid microcarcinoma(TMC),and to improve the early diagnostic rate.Methods:CDU diagnostic results and clinical data of 90 cases with TMC comfirmed by pathology from 1990 to 2002 were analyzed.Results:52 cases of the 90 patients were dianosed both by CDU and pathology (57.78%), 12 cases remained unclear dianosis,and 16 cases were misdiagnosed as benign lesion. The positive rate under ultrasonography with fine-needle aspiration cytology (FNAC) was 84.21%.Conclusion:Although CDU was limited in TMC diagnosis,the early diagnostic rate could be improved by combination of comprehensive analysis of the lesions′ images and choosing appropriate measurment such as FNAC.
