注意缺陷多动障碍患儿体感诱发电位的特征性改变和意义

摘要 目的 探讨注意缺陷多动障碍 (ADHD)患者体感诱发电位改变特征及其对脑功能损害程度及定位诊断的价值。方法 对 61例ADHD患儿实验组 ,2 7例精神发育迟滞 (MR)儿童 (病理对照 )和 2 3例健康儿童 (正常对照 )进行了体感诱发电位 (SEP)检测 ,观察各组成波的潜伏期和形态改变。结果 ADHD儿童与正常对照组儿童相比较 ,N2 、P2 、N3 等潜伏期延长、波形有缺失改变 ,右侧改变较左侧明显 ;MR儿童的SEP改变较ADHD更明显 ,且左侧改变较右侧明显 ,统计学处理有显著性差异 (P <0 0 5)。结论 ADHD的脑功能损害以皮层及皮层下为主 ,右侧损害重于左侧 ,MR儿童脑功能损害更严重 。 Objective To explore the characteristic changes of somatosensory evoked potentials (SEP) i n children with attentional deficit hyperactivity disorder (ADHD) and mental ret ardation (MR), and assess the value of SEP as a localization diagnostic tool on cerebral functional lesions. Methods The 61 children with ADHD (experimental group), 27 case s of MR (case control group) and 23 healthy children (normal control group) were measured for SEP. The latent periods and the forms of every constitutive wave o f SEP were analyzed. Results Compared with the normal control group, the N 2, P 2, N 3 latencies were prolonged significantly (P<0 05) in the children with ADHD and MR, and some waves were even absent. The changes of SEP on right hemisp here were more obvious than on left side in children with ADHD. The changes of S EP were more obvious in case control group than in experimental group, and in co ntrast, the changes of SEP on left hemisphere were more significant than those o n right side in the children with MR. Statistical analysis showed that the diffe rences were significant (P<0 05). Conclusion The functional abnormalities in ADHD were localized prominently in cerebral cortex and subcortex, and the changes of SEP were more m arked on right hemisphere than on the left side. The alterations in SEP were mor e prominent in case control group than in experimental group, and the left hemis phere was more severely suffered comparatively.
