摘要
目的 观察双滤过手术治疗青光眼的疗效。方法 选择 130只眼晚期、绝对期原发性青光眼随机分为两组 ,每组 6 5只眼。 组行双滤过手术 , 组行小梁切除术。结果 术后随访观察 5～ 2 1月 ,平均 (14 .34±4 .35 )个月。 组眼压≤ 2 1m m Hg(1m m Hg=0 .133k Pa) 6 2只眼 (95 .4 % ) ,其中需局部用药 6只眼 (9.2 % ) ,术后平均眼压为 (19.37± 2 .92 ) m m Hg,较术前下降 (33.2 6± 12 .33) m m Hg; 组眼压≤ 2 1mm Hg5 5只眼 (84 .6 % ) ,其中需局部用药 12只眼 (18.5 % ) ,术后平均眼压 (2 2 .6 2± 5 .6 8) m m Hg,较术前下降 (2 7.83± 8.81) m m Hg。两组手术成功率差异有显著性 (P <0 .0 5 )。并发症 : 组 9只眼 (13.8% ) ,其中浅前房 6只眼 (9.2 % ) ,前房积血 3只眼 (4 .6 % ) ; 组 2 2只眼 (33.8% ) ,其中浅前房 15只眼 (2 3.1% ) ,恶性青光眼 2只眼 (3.1% ) ,脉络膜脱离 2只眼 (3.1% ) ,前房积血 3只眼 (4 .6 % )。两组并发症的发生率差异有显著性 (P <0 .0 5 )。结论 青光眼双滤过手术效果好 ,并发症少 。
Objective To evaluate the clinical effect of double-filtering surgery for glaucoma.Methods 130 eyes with late or absolute stages of primary glaucoma were randomly divided into two groups.65 eyes of group Ⅰ were treated by double-filtering surgery and 65 eyes of group Ⅱ were treated by trabeculectomy.Results The follow-up after surgery was (14.34±4.35) months.In the group Ⅰ the intraocular pressure (IOP) of 62 eyes (95.4%) was ≤21mmHg(1mmHg=0.133kPa) and among them,there were 6 eyes(9.7%) with local pressure lowering agents.The mean postoperative IOP was (19.37±2.92)mmHg and (33.26±12.33)mmHg lower than pre-operation.In the group Ⅱ the IOP of 55 eyes (84.6%) was ≤21mmHg and among them,there were 12 eyes(18.5%) with medicines.The mean postoperative IOP was (22.62±5.68)mmHg and (27.83±8.81)mmHg lower than preoperation.Statistical significant difference (P<0.05) was found in the success rates between these two groups.The major complications of group Ⅰ were shallow anterior chamber in 6 eyes(9.2%) and a small amount of hyphema in 3 eyes(4.6%).The major complications of group Ⅱ were shallow anterior chamber in 15 eyes (23.1%),malignant glaucoma in 2 eyes(3.1%),choroidal detachment in 2 eyes(3.1%) and hyphema in 3 eyes (4.6%).Statistical significant difference(P<0.05) was found in the incidence of complications between two groups.Conclusion Using the double-filtering surgery may obtain good therapeutic results and less complications.Therefore,double-filtering surgery is a safe and effective method for the treatment of glaucoma.
