摘要
目的 探讨多层螺旋CT仿真血管镜的成像方法及临床应用价值。方法 回顾性分析 80例已行多层螺旋CT检查的病例 ,其中 6 0例为增强扫描 ,2 0例为平扫 ,将患者的原始扫描数据进行低对比强化 (ker nel≤ 30f)及薄层 (层厚及层间距均≤ 3.0mm)图像重建 ,然后把重建图像导入CT 3D工作站 ,利用机器所带软件Fly -Through通过调整阈值做仿真内镜观察并做出分析评价。 结果 6 0例增强扫描的患者均能获得大血管的类似内窥镜图 ,4 2例能显示到 2级分支血管腔镜图 ,8例获得 3级分支其以下血管腔镜图。发现动脉钙化及斑块 12例 ,动脉瘤 2例 ,血管畸形 3例 ,血管狭窄 4例 ;在图像质量和解剖细节方面 ,CT仿真血管镜能较横断图像获得更多的信息。结论 多层螺旋CT血管仿真内窥镜可比较直观地观察血管腔内的情况 ,显示管壁的钙化及揭示管腔狭窄的原因 ,其最大的优势是快速和无创伤性。
Objective: To study the practical imaging methods and evaluate clinical application of multi-slice helical CT of virtual intravascular endoscopy(MSCTVIE) .Methods:Eighty patients were performed in multi-slice helical CT volume scanning, including 60 patients were contrast enhanced and 20 patients were no contrast, processing the raw data with lowly kernel (kernel ≤ 30 f) and thin slice (slice≤ 3mm) reconstruction, then transmitted to CT 3D workstation, adjusting threshold values to process CTVIE in Fly-through imaging, and then analysed and evaluated them. Results: CTVIE imaging were successful completed in 60 patients of contrast enhancing. CTVIE images of 42 patients can display the inner lumen of second offset vessels, 8 patients can display after third offset vessels. Among them can show atherosclerosis(n=12), aneurysm(n=2), arteriovenous(n=3) and aortic stenosis(n=4). Comparing with axial images, CTVIE can show more information for imaging quality and specific anatomy.Conclusions: Multi-slice CTVIE images can display the inner lumen of vessels directly, and show calcification of vessels and reason of stenosis. The best advantage is more quickly and nonevasively.
