摘要
〔目的〕探讨疫情报告中HIV抗体初筛试验与确认 (WB)测试结果的符合情况。〔方法〕对初筛和复筛测试的阳性结果分成两组与确认 (WB)试验结果对比分析。第一组OD值与临界值 (cutoff)的比值 (S/CO)≥ 6 0 ;第 2组 (S/CO)比值在 1 0～ 6 0之间 ;同时观察两种初筛试剂复测结果一阴一阳与确认试验的符合情况。〔结果〕5 0 2份HIV抗体初筛阳性血清经确认 (WB)测试HIV抗体阳性 499份 ,HIV抗体阴性 2份 ,不确定血清 1份。复筛与确认 (WB)测试的第一组 (S/CO)比值≥ 6 0 ,HIV抗体阳性符合率为10 0 % ;第二组 (S/CO)比值在 1 0～ 6 0之间 ,HIV抗体阳性符合率为 84 61%。初筛假阳性 2份 ,假阳性率为 0 5 5 % ;复筛假阳性 1份 ,假阳性率为 0 3 3 %。两种初筛试剂测试结果一阴一阳的一份血样品经 (WB)确认为阴性结果。〔结论〕省级疾病控制中心艾滋病确认实验室在实验室质量管理制度和HIV抗体筛查符合《规范》要求的条件下 ,两次重复 (ELISA) (S/CO)比值≥ 6 0时 ,可以按HIV感染报告疫情。 (S/CO)比值在 1 0～ 6 0之间或复测结果一阴一阳 ,还应进一步作确认 (WB)。
Objective To study the consistent results between the primary screening test and reconfirmation test for HIV antibody in epidemic situation report. Methods Divide the positive results of the primary screening test and the second screening test into two groups, which are then compared with the results of the reconfirmation test(WB).In the 1st group the ratio of OD and cut off (S/CO) is over or equal to 6.0(≥6.0);In the 2nd group the ratio of S/CO is between 1.0 and 6.0;at the same time observe two samples in the primary screening test whose results in the second test are positive and negtive; and the result of the reconfirmation test. Results The reconfirmed tests of Weston Blot were done in 502 serum samples which are positive in the primary screening tests. The results were 499 postive,2 negative and 1 questionable. Contrasted with the primary screening test, the check ratio of positive is 100%(S/CO ≥6.0),and 76.92%(1.0< S/CO <6.0).The rate of the fake positive is 0.5% in the primary screening test and 0.33% in the second screening test. Conclusion HIV testing technologies and the quality administer system of the HIV reconfirmation laboratory are according to the National HIV/AIDS Testing Technologies Creterion in the center for disease prevention and control in Henan province. When the serum is tested two times with the primary screening test of ELISA or S/CO exceed 6.0,it can be reported positive. But when the S/CO is from 1.0 to 6.0 or the results aren't similar in two times primary screening test ,the serum must be tested with the reconfirmation test of Weston Blot.
