胶质瘤微血管及瘤周水肿改变与MRI相关性研究

摘要 目的 探讨胶质瘤血管成份中血管内皮生长因子 (VEGF)及其受体mRNA表达改变与瘤周水肿MRI的关系及肿瘤增殖能力对血管成份的影响。方法 采用半定量逆转录 聚合酶链反应 (RT PCR) ,对 49例脑胶质瘤手术切除标本 ,U2 51等 3株胶质瘤细胞、人类脐静脉内皮细胞 (HUVEC)以及 12例脑外伤内减压术中得到的正常脑组织检测了VEGF、mRNA表达水平。另外 ,用免疫组化SP法对胶质瘤及正常脑组织的微血管密度 (MVD)并与星形胶质细胞瘤MRI、病理学资料进行分析。结果 胶质瘤组织中的VEGF、mRNA表达水平明显增高达 65.3 % ,随着病理分级的提高其表达水平有升高趋势 ,HUVEC及 3株胶质瘤细胞表达水平均较高 ,而正常脑组织表达水平及表达率较低。胶质瘤中的微血管密度 (MVD)与胶质瘤级别之间有等级相关性 ,VEGF、mRNA的表达同MVD呈正相关。病理级别增加 ,MRI评分值亦随之增加 (P <0 .0 1~ 0 .0 0 1)。增强形态 (CE HET)、瘤周水肿及增强程度 (CE D)与病理级别良、恶性程度密切相关 (P <0 .0 0 1)。结论 VEGF、MVD对胶质瘤的恶性生物学行为的评估有重要意义 ,星形胶质细胞瘤的微血管成份改变对MRI增强程度、瘤周水肿、信号不均匀、增强形态有明显影响 ,血管丰富、病理级别高的肿瘤易伴瘤周水肿 ,而MRI强化明显 。 Objective To study correlations between the changes of t he expression of vascular endothelial growth factor(VEGF) as well as its recepto rs mRNA in gliomas, vascular components in astrolytoma and MRI features, and th e influence of the proliferative activity of tumors on the vascular components. Methods The mRNA level of VEGF and its receptors in surgical sa mples of 49 patients suffering from glioma one examined using semi-quantitative reverse transcription polymerase chain reaction(RT-PCR). 3 glioma c ell lines , the human umbilical vein endothelial cells(HUVEC) and 12 samples of normal brain tissues obtained from cranicerebral injury surgery are studied as c ontrol. Immunohistochemical SP method was used to analyze the intratumor micro vessel density(MVD)with MRI and pathological data Results The expression level of VEGF, mRNA was significantly high i n gliomas,positive rate s were 65.3%. The expression level paralleled with the grade of glioma . HUVEC a nd 3 glioma cell lines also expressed high level of three factors, but the norma l brain tissues remain lower expression. MVD increased with glioma pathologi c grades. There was a positive correlation between mRNA level of VEGF and MVD. MRI score increased with pathological grades ( P <0.01~0.001) Contrast enhancement heterogeneity (CE-HET) thus proved to be related to the pathological grade by a multiple regression analysis( P <0.001).Concl usion VEGF, mRNA have significance to evaluate the malignant b iological behavior of glioma cells as well as MVD. Change of the vascular components of gliomas had the obvious influences on peritumoral edema, CE-D, CE-HET. Tumors with rich vascular components are o ft en accompanied by peritumoral edema and CE-D while tumors with proliferativ e activity were often with obvious vasculogenesis.
