摘要
目的:探讨无牙颌不同颌位时颌前点的变化规律,为无牙颌修复时位的确定提供根据。方法:选取临床上22例无牙颌重建的患者,在五组颌位上拍摄头颅侧位片,分别测量颏前点到圆点的距离和SNPg角的数值及在正中关系位时SN-GoGn角的数值。结果:各组颏前点到圆点的距离无显著差异。无牙颌建位与正常人位无显著差异,从无牙颌位至最大张口位下颌突度的变化差异显著。结论:下颌颏部从无牙颌位到最大张口位有其自身的运动规律,颏前点的运动轨迹为围绕某一点旋转的弧形。无牙颌重建时协调的长正中关系位就位于此弧线上。从无牙颌位到最大张口位,下颌颏部突度的变化成不连续性减小。从无牙颌位到正中关系位下颌突度减小;从3/4张口位到最大张口位下颌突度无变化。
Objective: To study the relationship between the different pogonions and cranium for the cletermination ofintercuspal position of edentulous patient. Methods: Cephalometric roentgenographs were made for 22 edentulous patientsof occlusal reconstruction in five positions. We measured the distances from Pg to centric point, the values of the SNPgangles in five positions. Results: In the five positions, there was no marked difference at 5% level of significance from Pgto the centric point and there were no statistical difference at 5% level of significance between occlusal reconstruction ofthe edentulous jaw and occlusal reconstruction of population. From the edentulous position to the largest open position,the mandibular protrusion showed marked difference at 5% level of significance. Conclusions: The results demonstratethat the mental part of mandible has its own law of movement from the edentulous position to the largest open position.The movement track of the Pg is like a curve and the position of long medianum is in this curve when it is reconstructedocclusion. The changes of the mandibular protrusion do not reduce continuously. The changes of the mandibular protrusionreduce from the edentulous position to the centric relation position. The mandibular protrusion does not change from the3/4 open position to the largest open position.
