摘要
取感染性休克23例测定空腹股动静脉血浆FAA(19种),并与正常人(n=30)及重症肺心病(n=19)对照研究。结果显示感染性休克较正常人有10种氨基酸明显降低,尤以生糖氨基酸与BCAA明显,提示糖异生的增强。感染性休克与重症肺心病显示相似的氨基酸模式,随着应激的加重BCAA/AAA下降明显、PHE/TYR比升高显著,PHE在血浆不成比例的升高是高分解代谢状态和肝脏受损的早期征象。
In 23 patients with septic shock, plasma free amino acids (AA) drawn from femo-ral artery and vein were determined. The data were compared with those of normalsubjects (n=30) and severe cor pulmonale group (n=19). The result showed that thelevels of 10 kinds of AAs were significantly lower in patients with septic shock thanthose in normal subjects, especially the BCAA and glycogenic AAs. It was suggestedthe rate of glycogenesis was increased in patients with septic shock. Septic shock wassimilar to severe cor pulmonale in plasma free AA pattern. The more the patients con-dition worsened, the more the ratio of BCAA/AAA lowered and the ratio of PHE/TYRelevated. PHE in plasma was not proportionally elevated and it was revealed an ear-lier feature of the hypermetabolism and liver impairment.
