摘要
目的 :探讨吻合器痔上黏膜环切术 (PPH)对 度～ 度痔病治疗术后并发症的成因及对策。方法 :总结 PPH治疗 85例 度～ 度痔病的临床资料。结果 :本组手术时间 2 0 min,住院 4 .5 d;术后脱出肛门外的痔块回缩有效率为 1 0 0 % ,5 2例(6 1 .2 % )患者的外脱痔块完全回缩 ,33例 (38.8% )残留外痔赘 ,2 0例 (1 7.9% )吻合切除术中再检查需要缝合止血 ,术后出血率为 2 .4 % ,35例 (4 1 .2 % )肛门疼痛需要止痛剂 ,2 0例 (2 3.5 % )精细控便障碍或大便失禁 ,1 2例 (1 4 .1 % )尿潴留病例。结论 :减少并发症的关键为手术操作平面为距齿线上 4～ 5 cm直肠黏膜 。
Objective:To evaluate and analyze the postoperative complications induced from the operative of procedure for prolapse and hemorrhoids(PPH).Methods:Analyzing the data of the third or fourth degree hemorrhoids operated by procedure for prolapse and hemorrhoids.Results:Total 85 patients with thied or fourth degree hemorrhoids were operated on by PPH,and the mean operative time was 20 minutes and mean postoperative hospital stay were 4 5 days.Postoperative bleeding was not found in any case.Conclusion:The procedure for prolapse and hemorrhoids(PPH) was safe,effective and rapid,causing minimum postoperative pain and helping patients return earlier to notmal activity.The key step to lower the rate of postoperative complications of PPH was the place and plane of circumferential mucosectomy. Purse string suture is the important factor that affects the effect of the operation.
