摘要
目的 :探讨微创治疗高血压脑出血的方法。方法 :采用自制简易定位装置 ,借助 CT扫描定向穿刺血肿 ,进行抽吸、尿激酶灌洗治疗。结果 :35例血肿 30~ 60 ml30例 ,61~ 1 0 0 ml4例 ,1 30 ml1例 ;一次性清除 3例 ,一次性清除 80 %以上 1 5例 ,60 %~ 80 % 1 7例无手术死亡。随访 2 4个月 ,恢复良好 38% ,轻残 5 0 % ,重残 1 2 %。结论 :微创血肿清除是一种降低病死率、致残率有效的治疗方法。
Objective:To study the treatment of hypertensive intracerebral hematomas under the operative microscope. Methods:Using simple equipment which was made by ourselves, hypertensive intracerebral hematomas was removed by CT-guided aspiration and infusion of urokinase. Results:CT-guided aspiration of hypertensive intracerebral hematomas was performed in 35 patients.The volume of hematoma was 30~60ml in 30 cases, 61~100ml in 4 cases, 130ml in 1 case.The liquid hematoma was totally aspirated in 3 patients, more than 80% of clot was evacuated first in 15 patients and 60%~80% of clot in 17, no operative death. Follow up 24 months in the study,38% were of patient's condition good,50% were mildly disabled and 12% were severely disabled. Conclusion:Microsurgical removal of hypertensive intracerebral hematoma is safe and effective and it can reduce the mortality and morbidity.
