A Fetal Lamb Model of a Closed Circuit Cardiopulmonary Bypass

摘要 目的探讨胎羊密闭式体外循环(CPB)的可行性。方法5头胎羊正中开胸,经主肺动脉插10F动脉管,右心房插12Fr~14Fr静脉管。离心泵连接动、静脉插管,形成密闭体外环路。CPB包括离心泵和膜式氧合器,静态预充量约100ml。转流开始后,阻断脐带血流。常温转流30min,灌注流量(189±29)mL/(kg·min)。恢复脐带血流,逐渐停机。监测胎羊血压、心率、血气等数据。结果胎羊体重(1.90±0.42)kg。转流中胎羊血压、心率稳定,血气结果在生理范围内。转流后1h,pH值下降,二氧化碳分压增高,与转流前比较差异有显著性,P<0.05。结论胎羊密闭式CPB方式减少CPB预充量,满足胎羊机体灌注。但是CPB对胎盘长期功能不利。 OBJECTIVE To study the establishment of fetal lamb cardiopulmonary bypass with a closed circuit. METHODS Five fetal lambs underwent median sternotomy. A 10Fr arterial perfusion cannula was inserted into pulmonary artery and a 12Fr ~ 14Fr venous cannula was put into right atrium. A closed circuit was composed of a centrifugal pump and artificial oxygenator. The total volume of priming was about 100ml. The umbilical vessels were clamped as bypass began. The normothermia bypass lasted 30min, undamped the umbilical vessels and finished the bypass. The fetal heart rate, blood pressure, arterial blood gas were assayed. RESULTS Fetal lamb weighted ( 1.90±0. 42 ) kg and perfusion flow rate were (189±29)ml/( kg. min). The fetal heart rate and blood pressure were normal during experiments. The values of arterial blood gas of fetal lamb were at the physical levels. The pH value decreased significantly and PCO2 increased remarkably after bypass lh( P <0. 05). CONCLUSION Fetal lamb cardiopulmonary bypass with a closed circuit can reduce the priming of circuit and maintain the fetal lamb perfusion well. However, cardiopulmonary bypass is detrimental to placental long - term functions.
