茶硷抗哮喘气道炎症作用的临床研究 被引量:1

摘要 目的 :观察茶硷对哮喘患者GM -CSF、EOS及FEV1 的影响 ,探讨茶硷类药物抑制气道炎症的作用机制。方法 :研究对象随机分为 :正常组 30例 ,茶硷组 2 4例 ,地塞米松组 2 4例 ,分别测定实验前及用药 2周后各组血清GM -CSF、外周血EOS及FEV1 。结果 :哮喘急性发作期 ,茶硷组和地塞米松组GM -CSF、EOS水平都显著高于正常组 (P <0 .0 1)。用药 2周后 ,茶硷组和地塞米松组GM -CSF、EOS、FEV1 3项指标分别较各组治疗前改变有显著性差异 (P <0 .0 1) ,但两组治疗后结果相互比较差异无显著性 (P >0 .0 5 ) ;茶碱组治疗后血GM -CSF、EOS水平显著高于正常组 (P <0 .0 5 ,P <0 .0 1) ;地塞米松组治疗后EOS水平显著高于正常组 (P <0 .0 1) ,GM -CSF水平与正常组比较差异无显著性 (P >0 .0 5 )。结论 :茶硷对哮喘的有效治疗可能是部分依赖于抗气道炎症作用 ,其机制可能与抑制EOS、GM -CSF有关 ;哮喘症状缓解的患者气道炎症仍存在 ,提示在哮喘治疗中 。 Objective: To investigate the mechanism of theophylline inhibiting airway inflammation. Methods:The experimental objects were divided into three groups randomly, normal group 30 cases, theophylline group 24 cases, and dexamethasone group 24 cases. The GM-CSF, EOS,FEV1were measured before and zweeks after the drug administration respectively.Results: Daring acate attack the levels of GM-CSF and EOS in theophylline group and dexamethasone group (0.27±0.08 and 325.92±51.43, respectively )were significantly higher than those in normal group (0.08±0.08 and 89.67±28.32, respectively, P<0.01).2 weeks after the therapy, the levels of GM-CSF, EOS and FEV1 in theophylline group and dexamethasone group decreased significantly (P<0.01); but there was no significant difference between the two patient groups.(P>0.05); and the levels of GM-CSF, EOS in theophylline group were higher than those in normal group(P<0.05; P<0.01 respectively).The EOS count in dexamethasone group was higher than that in normal group (P<0.01 ), the level of GM-CSF in dexamethasone -treated group was not significantly higher than that in normal group (P>0.05).Conclusion: Theophylline can inhibit airway inflammation in asthma through inhibiting the release of GM-CSF and inducing the activity of EOS.
