体外循环技术对建立绵羊的心脏瓣膜植入实验模型的影响 被引量:5

摘要 目的 :探讨体外循环技术对建立绵羊的人工瓣膜植入实验模型的影响。  方法 :8只雄性绵羊左侧开胸 ,升主动脉插动脉管。右心耳和 (或 )上腔静脉插静脉管 ,并行循环维持心跳下将国产人工机械瓣植入到肺动脉瓣位及二尖瓣位。  结果 :8只绵羊中 7只存活 (超过 1个月 ) ,1只因低心排血量手术死亡。平均转机时间 (70 0± 11 0 )min ,平均预充新鲜血量 (575 0± 2 65 9)ml ,平均追加血量 (175 0± 198 2 )ml,平均转中最低鼻温 (2 8 8± 2 9)°C。早期轻度肺部并发症4只 ,经做相应处理均痊愈。  结论 :采用体外循环并行下手术的基本方法可顺利完成人工瓣膜的植入。掌握好几个关键的体外循环技术环节 ,可有效地提高绵羊的人工瓣膜植入实验模型的成功率。 Objective: The purpose of this study was to investigate the effect of card iopulmonary bypass technique on sheep implanted mechanical valve model. Methods: The mechanical valves were implanted in eight sheep under cardiop ulmonary bypass with a beating heart through the left thoracotomy. The cardiopul monary bypass was establised by arterial cannulation via ascending aorta and ven ous cannulation via right atrial appendage and(or) superior vena cava. There were 2 cases of mitral prosthesis in mitral valve position and 6 cases of aortic p rosthesis in pulmonary valve position. Results: One of 8 sheep died from low cardiac output syndro me at operation. The mortality was 12 5%. The other 7 survived more than one month. The m ean pu mp time was (70 0±11 0) min. The mean priming volum e of fresh blood was (575 0±26 5 9) ml. The mean adding volume of fresh blood was (17 5 0±198 2)ml and the lowest mean nasal temperature was (28 8±2 9)°C during the pumping. Four sh eep had mild pulmonary complications and were cured by reasonable treatment. Conclusion: The implanted mechanical valve procedure can be satisfactorily performed under the cardiopulmonary bypass with a beating heart. The success rate of implanted mechanical valve sheep model may be properly improve d by dealing with several key links of cardiopulmonary bypass technique.
