不同原因食管狭窄的内镜治疗 被引量:1

摘要 目的 :探讨微波、气囊扩张及高频电切对食管狭窄的治疗效果。方法 :通过胃镜检查明确食管狭窄的性质、部位及程度。根据狭窄的性质、部位及程度不同 ,采用内镜下气囊扩张或和微波及高频电切治疗。结果 :95例食管狭窄中 ,术后吻合狭窄 35例 ,放疗术后狭窄 2 1例 ,癌性狭窄 2 4例 ,化学烧伤后狭窄 1 3例 ,不明原因性狭窄 2例。其中微波治疗 5例 ,电切 +微波治疗 3例 ,气囊扩张治疗57例 ,微波 +气囊扩张治疗 30例。治疗 1~ 43次。显效 72例 (75 79% ) ,有效 2 1例 (2 2 1 1 % ) ,无效 1例 (1 0 5% ) ,食管穿孔 1例(1 0 5 % )。结论 :根据食管狭窄的性质、部位及程度不同 ,分别或联合采用内镜下气囊扩张、微波及高频电切治疗 ,方法简单、价廉、安全、有效 。 Purpose: The therapeutic effects of microwave coagulation,balloon dilatation and elctrocoagulation resection for esophageal stenosis were evaluated. Methods: The esophageal stenosis was diagnosed by endoscope.Four kinds of endoscopic therapy including microwave coagulation,elctrocoagulation resection+microwave coagulation,ballon dilatation and microwave coagulation+balloon dilatation were selected according to the different reasons and serious degree of esophageal stenosis. Results: 95 esophageal stenosis were caused by anastomotic site stenosis after esophagectomy 35 cases,radiotherapy 21 cases,esophageal cancer 24 cases,caustic damage 13 cases and unclear reason 2 cases.Of 95 cases esophagostenosis,5 cases were treated by microwave coagulation,3 cases by elctrocoagulation resection+microwave coagulation,57 cases by balloon dilatation and 30 cases by microwave coagulation+balloon dilatation.93(97.89%) esophageal stenosis were improved,1 did not change and 1 occurred esophageal leaks after endoscopic treatment. Conclusions: Endoscopic therapies including microwave coagulation,elctrocoagulation resection+microwave coagulation,balloon dilatation and microwave coagulation+balloon dilatation were effective to esophageal stenosis caused by any reason.
