摘要
目的探讨显微支撑喉镜下CO_2激光治疗早期喉恶性肿瘤的临床疗效。方法经术前病理诊断和术后病理证实的13例早期声门癌(T_1N_0M_0)及33例喉白斑,于全身麻醉下在显微支撑喉镜下行3种类型激光声带术治疗。结果经平均24个月随访,46例中除开展此术初期的2例喉白斑因阴性切缘不足,分别于术后3个月和5个月复发而重行激光术外,其余病例无并发症及复发征象,发声功能恢复良好。结论显微支撑喉镜下CO_2激光声带术是对早期喉恶性肿瘤行之有效的微创手术,既能达到根治,又能保护发声质量。
Purpose To investigate the clinical effectiveness of the endoscopic and microscopic CO2 laser surgery for managing early glottic carcinoma and laryngeal precancerous lesions. Methods Thirteen patients with stage I glottic carcinoma (T1N0M0)and thirth three patients with laryngeal precancerous lesions were operated on under endoscope and microscopic CO2 laser surgery. All cases were diagnosed pathologically, and the operation preformed could be differentiated into three pateems, namely Type Ⅰ , Ⅱ and Ⅲ . Results Data of follow-up for average 18 months showed that excellent local control was gained in all cases with the exception of two cases of laryngeal keratosis. These two who recurred 3 and 5 months after operation respectively were treated by laser surgery again. It at least indicated that short-term effect of the treatment was satisfying. Conclusion It is effective to treat early glottic carcinoma and laryngeal precancerous lesions with endoscopic and microscopic CO2 laser micro-traumatic surgery. By such an operation we could not only ablate the cancer radically, but also improve the postoperative voice quality.
