摘要
目的 探讨功能性肠病 (FBD)住院发病情况、临床特点及住院耗资。方法 复习我院消化科 1999～ 2 0 0 2年期间住院的FBD和器质性肠病 ,比较两大类疾病临床特征、辅助检查及住院费用情况。结果 FBD 65例 ,器质性肠病167例。两组患者腹痛、腹泻、腹胀、便秘、厌食、乏力、体重下降均无显著性差异 (P >0 .0 5 ) ,FBD多伴随焦虑、抑郁和失眠 ,而便血、发热、潮热、盗汗、贫血、腹水征、腹部包块多提示器质性肠病。FBD多数检查异常发现率及平均每日住院费用较器质性肠病低 (P <0 .0 5 )。检查费用两组无显著性差异 ,而检查费用占住院费用比例FBD较器质性肠病高 (P <0 .0 5 )。结论 临床医师应提高对FBD认识 ,熟悉诊断标准 。
Objective To investigate the clinical manifestations and medical expenses of patients with functional bowel disorders(FBD)in hospital.Methods The clinical manifestations,assistant examinations and medical expenses of FBD and organic bowel diseases diagnosed in our hospital from 1999 to 2002 were compared.Results Sixty five patients were FBD,and 167 patients were organic bowel diseases.There were no differences in abdominal pain,diarrhea,abdominal bloating,constipation,anorexia,inertia and weight loss between two groups( P >0.05).The patients with FBD were often anxious,depressed and insomnia.Hematochezia,fever,hectic fever,night sweat,anemia,ascites and masses in abdomen were suggestive of organic bowel diseases.The abnormal occurrences detected by assistant examinations and hospitalization cost per day in patients with FBD was lower than those in patients with organic bowel diseases.There was no difference in cost of examinations.The ratio of examinations cost to hospitalization cost in patients with FBD was higher than that in patients with organic bowel diseases.Conclusion Clinical doctors should pay great attention to FBD,be familiar with diagnosis criterion and select assistant examinations reasonablely.
