摘要
目的:观察不同保守方法治疗幼年髁状突纵形骨折对髁状突软骨的影响,为临床治疗提供依据.方法:幼年小型猪26头随机分为正常对照2头及髁状突纵形骨折后未洽疗组、(牙合)夹板治疗组、颌间固定组及(牙合)夹板结合颌间固定组各6头.实验后3、6、12周取材光镜观察.结果:(牙合)夹板治疗12周时,骨折愈合,部分软骨再生;SFMC未治疗出现盘突粘连,呈双髁突畸形;颌间固定治疗与(牙合)夹板结合颌间固定3周,关节盘变薄,破裂.并随固定时间延长出现盘突与颞下凹粘连,软骨变薄、变性.结论:(牙合)夹板治疗幼年髁状突纵形骨折有利于骨折愈合及软骨再生,颌间固定治疗时间宜短,并结合(牙合)夹板使用.
Objective: To observe the effects of varied conservative treatments on condylar cartilage after sagittal fracture of mandibular condyle(SFMC). Methods: 26 young minipigs were divided randomly into normal control group and: ①no treatment(n=6), ②Occlusal Splint treatment(n=6), ③Intermaxilary Fixation treatment(n=6), ④Occlusal Splint & Intermaxillary Fixation treatment(n=6) after SFMC were performed. The specimen were made at 3,6,12 weeks after operations and light microscopes were applied for observations. Results: There were no adhesion and deformity within TMJ after SFMC with Occlusal Splint treatment. The fractured bone had healed and partial cartilage had regenerated. Adhesions occurred between the disc and condyle and bifid deformity of the condyle were formed after SFMC. The disc were worn and there were adhesions between disc and condyle when treated with Intermaxilary Fixation either with and without Occlusal Splint at 3 weeks. Conclusions: Occlusal Splint treatment can prevent the adhesions within TMJ, do benefit to fracture healing and cartilage regeneration. IMF should be used with OS in a short term of time.
