羟基磷灰石涂层种植体临床应用现状的系统评价 被引量:1

摘要 目的 :Meta 分析法研究羟基磷灰石 (HA)涂层种植体的临床应用现状 ,评估HA涂层种植体修复的总体平均成功率。方法 :计算机检索 1990 - 2 0 0 2年国内所有口腔医学类杂志有关涂层种植体修复的文献 ,根据纳入和排除标准 ,分两步选择适用文献 14篇进行质量评价。对种植体成功标准作了统一的定义 ,作为统一的研究结果。对文献进行均衡性检验 ,把相关文献数据合并计算出HA涂层种植体修复的总体平均成功率。结果 :所有文献按 0~ 1标准的平均加权质量分为 0 .6 1± 0 .0 6 ,各篇文献的质量分值分别作为其合并样本量的权数。计算临床成功率的 11篇文献 ,观察时间 2年以下 ,2~ 4年和 4年以上所得总体临床成功率的均值分别为 98.16 % ,94 .73% ,92 .0 2 % ;通过寿命表法获得累积成功率的 3篇文献随访 1,2 ,3年的总体平均累积成功率分别为 95 .1% ,92 .5 1% ,91.0 5 % ,由于 10号文献的随访时间只有 3年 ,3篇文献 3年后的累积成功率无法合并。结论 :HA涂层对骨内种植体的临床成功率有影响 ,HA涂层种植体修复近期的临床成功率及累积成功率较高。 Objective: Our aim was to study the clinical application of hydroxyapatite coated implant by using a Meta analysis review procedure Methods: Using a systematic computer search for the Chinese literature from the dental journals in China from 1990 to 2002, papers were selected with an in and exclusion protocol in a two step procedure. In addition, the 14 papers selected were subjected to a quality assessment. All studies used an identical definition of success standard of hydroxyapatite coated implant. After a homogeneity test, the mean overall survival rates were calculated by pooling the data from the studies. Results: On a 0~1 scale, the weighed overall mean quality score of the studies was 0.61 (s.d.0.06), the quality score was used as weight for pooling the samples. There were eleven studies reported implant survival using overall percentage, the overall mean survival rates for 2 years below, between 2 to 4 years, 4 years above were 98.16%,94.73%,92.02%,respectively. In other 3 studies reported implant survival with life table analysis, the mean cumulative survival rates for 1 year,2 years, 3 years were 95.1%,92.51%,91.05%, respectively. Because the follow up time of one study was only 3 years, their cumulative survival rates after 3 years could not be pooled. Conclusion: The HA coatings have an effect on survival rates on endosteal implant. The recent survival rates of HA coated implant restorations are high. Howewer, because the follow up time is short, the long term survival rates need further studies.
