子宫腺肌症的MRI与经阴道超声影像学分析及评价 被引量:4

摘要 目的 :探讨子宫腺肌症的MRI与经阴道超声影像学特点以及其诊断价值。材料和方法 :分析 16例MRI/经阴道超声 (TVS)诊断而经手术病理证实的子宫腺肌症的影像学特点和两种影像检查手段的特异性、敏感性及准确性。结果 :MRIT2WI与脂肪抑制成像示所有患者子宫结合带厚度均大于 12 .0mm ,平均 16.4mm ;子宫肌层内信号减低区斑点状高信号 ( 14例 )。经阴道超声二维显像示子宫肌层回声减低区内更低回声的小腔隙以及彩色多谱勒示病变区血流丰富 ( 10例 ) ;MRI诊断的特异性、敏感性及准确性均为 10 0 % ,经阴道超声分别为 10 0 %、62 .5 %及 62 .5 %。结论 :对于子宫腺肌症的诊断 ,MRI与经阴道超声有相应的影像特征及较高的特异性 ,但MRI的敏感性及准确性高于经阴道超声。 Purpose: To investigate MRI and TVS manifestations of uterine adenomyosis and to evaluate their diagnostic value in the disease. Materials and Methods: 16 cases of uterine adenomyosis were evaluated by MRI and TVS. Results: MRI showed all the junctional zones in adenmoyosis were thicker than 12.0mm, mean 16.4mm and spotly high signal in a abnormal low signal zones in 14 cases on T2WI and fat-suppression imaging. The more hypoechoic lacunaes existed in the hypoechoic myometrium on two dimensional TVS and the increased blood stream in the foci on CDFI in 10 cases. The specificity, sensitivity and accuracy of MRI in diagnosis of uterine adenomyosis were all 100%, and that of TVS were 100%, 62.5% and 62.5% respectively. Conclusion: MRI and TVS have some imaging characters for diagnosing adenomyosis, but the sensitivity and accuracy rate of MRI is higher than TVS.
