摘要
目的 探讨组织多普勒成像 (TDI)检测胎儿房室环的运动特征及其随孕龄变化情况的可行性。方法 应用TDI技术的彩色二维和脉冲方式观察 6 7例正常胎儿四腔心切面的二、三尖瓣环运动 ,测量其收缩、舒张期运动的峰值速度 ;用传统多普勒显像测量舒张期二、三尖瓣口血流峰值速度 ,并与舒张期房室环的运动测值进行比较。结果 二、三尖瓣环收缩期及舒张早期峰值速度均随孕龄显著增大 ,舒张早、晚期房室环运动峰值速度之比 (Ea/Aa)亦随孕龄显著增大 ,舒张晚期二、三尖瓣环峰值速度随孕龄增加的幅度较小 ;二尖瓣环的峰值速度明显小于三尖瓣环 (P <0 .0 1) ;Ea/Aa与瓣口血流峰值速度之比E/A相关性较好。结论 TDI可用于检测胎儿二、三尖瓣环的运动及其随孕龄的变化 ,并可作为一种新的评价胎儿心功能的方法。
Objective To assess the feasibility of tissue Doppler imaging (TDI) in normal fetuses,and to analyze peak velocities of the mitral annulus (MA) and the tricuspid annulus (TA) in different gestational ages.Methods Tissue Doppler images were obtained in 67 normal fetuses between the 17th and the 41th week of gestation. The subjects were divided into three age groups. Measurements included peak velocities of MA and TA during systole,early diastole and late diastole,and the ratio of peak velocities during early diastole to that during late diastole (Ea/Aa).Results Systolic and early diastolic peak velocities and Ea/Aa of both MA and TA increased significantly with the increasing of gestational age,but the correlations between late diastolic data and gestational age were weaker. The peak velocities of MA were significantly lower than those of TA (P<0.01).In both MA and TA,there were significant relation between Ea/Aa and pulsed Doppler E/A (P<0.01).Conclusion TDI is useful in assessing the motion of MA and TA in fetuses and observing age-related alterations.TDI is a new method for assessing the fetal cardiac function.
