尿8种超微量蛋白与管型蛋白成份分析在肾脏及肾脏相关疾病中的应用 被引量:1

THE USE OF ELISA IN THE DETERMINATION OF 8 URINARY TRACE PROTEINS AND THE PROTEIN COMPOSITION OF CASTS IN PATIENTS WITH RENAL DISEASE AND RENAL ASSOCIATED DISEASES

摘要 用ELISA法检查尿中自蛋白(Alb)、Tamm-Horsfall糖蛋白(THP)、IgG、IgA、IgM,C_3、α_2巨球蛋白(α_2M),纤维结合蛋白(F_n)含量,同时用酶免疫染色法分析尿管型中的蛋白构成,发现正常人运动后尿Alb、IgG、IgA升高,但仍在正常范围,运动、酸负荷后管型中除THP外偶见Cα_3。高血压、糖尿病患者在控制血压或血糖水平后尿中升高的Alb、Ig可恢复,管型中Alb,Ig应消失,否则提示有肾脏病变。肾脏病患者8种尿蛋白均增加,管型中可出现多种蛋白成份,即使亚临床患者也不例外。以上结果提示,此分析法是一种有助于早期预测肾功能的非创伤性检查法。 The ELISA technique which is sensitive to ng/ml level was used to determine urinary trace AIb,THP(tamm-horsfall glycoprotein),IgG,IgA,IgM,C3,a2 M(a2-macroglobulin),Fn(fibronectein).At the same time an immunoenzyme stain method was used to analyse the protein composition of urinary casts.The results are as follows:(1) in healthy subjects,urinary Alb,IgG,IgA level were increased after they had taken exercise or NH4Cl,but still in normal range,while THP (occasionally C3) was the exclusive component in urinary casts.(2) in essential hypertension or diabetic patients,increased urinary Alb,Ig and Alb,Ig componnet appeared in casts would be lowered to normal level or even disappeared when the underlying diseases had been controlled;the contrary indicated that renal damage had taken place.(3) in patients with glomerulopathy (including subclinical lupus nephritis) or tubular-interstitial kidney diseases,all 8 urinary proteins were increased significantly,and they were also discovered in urinary casts.The above results suggested that this analysis might be helpful to monitor the renal function.
