摘要
目的 探讨Paget病的临床及病理形态学特点 ,提高对Paget病的诊治水平。 方法 回顾性分析 9例Paget病患者的临床资料及病理资料。结果 9例Paget病 ,男性患者 4例 ,女性 5例 ,年龄 40～ 82岁 ,平均 63岁。发生部位 :乳头乳晕 5例 ,乳房外 4例。镜下Paget细胞呈巢样、腺样或散在分布于表皮内。细胞体积大 ,呈圆形或椭圆形 ,胞质丰富而透亮 ,核大深染 ,核分裂相易见。免疫组化结果 :EMA、CK 7阳性 ,HMB 45阴性。结论 该病的诊断主要依据组织病理及免疫组织化学检查 ,手术是惟一有效的治疗措施 ,病灶必须彻底切除 。
Objective To investigate the clinicopathological characteristics of Paget′s disease and improve the diagnosis and treatment result.Methods Nine patients with Paget′s disease were retrospectively reviewed.Results Four male and five female patients,aged from 40 to 82 years with an average of 63 years,included five cases of mammary Paget′s disease(MPD) and four cases of extramammary Paget′s disease(EPD).The paget′s cell appeared in a form of nest,gland or scattered in epidermis.The tumor cells were big and the cytoplasm was rich and clear.EMA and CK7 were positive and HMB-45 was negative in all cases.Conclusion Paget′s disease is diagnosed by histopathological method and immunohistochemistry.Surgery is the only effective treatment for Paget′s disease.Complete removal of the tumor may help to prevent local recurrence.
