摘要
对12例机械通气肺心病患者经博利康尼雾化治疗后对心肺功能的影响进行了研究。在生理盐水雾化后30分钟与5mg博利康尼雾化治疗后30、60和120分钟分别测定了气道峰压、间歇压、右房压、肺动脉压、肺动脉楔压、心输出量、动脉和混合静脉血气。结果表明,与生理盐水雾化比较,博利康尼雾化治疗后气道峰压、气道阻力、肺动脉压力、肺血管阻力明显降低,同时心率、心脏指数、氧运输指数和混合静脉血氧分压明显增加。说明博利康尼雾化治疗可明显改善机械通气肺心病患者的呼吸动力学、血流动力学和组织供氧,且伴有心率的增加。
Cardiopulmonary effects of nebulized bricanyl were studied in 12 mechanically ventilated patients with cor pulmonale. Measurements of peak airway pressure, pause airway pressure, right atrial pressure, pulmonary ariery pressure, pulmonary artery wedge pressure, cardiac output, artery and mixed venous blood gas were made at 30 minutes after nebulized normal saline and at 30, 60, and 120 minutes after nebulized bricanyl treatment. In comparison with normal saline, bricanyl resulted in a significant decrease of peak airway pressure, airway resistance, pulmonary artery pressure and pulmonary vascular resistance. However, significant increase of heart rate, cardiac index, oxygen delivey index and mixed venous oxygen pressure was simultaneously observed. It indicates that nebulized bricanyl could improve respiratory mechanics, hemodynamics and oxygen supplement of tissue in mechanically ventilated patients with cor pulmonale, but it increased heart rate.
