摘要
目的 :探讨肝前性门静脉高压症的临床特点、诊断和治疗方法的选择。方法 :经造影和手术发现非肝硬化性门静脉纤维化 3例 ,肝外门静脉阻塞 9例 ,临床表现除具有与肝硬化门静脉高压症相似表现外 ,尚具有年龄轻 ,肝功能化验正常等特点。治疗采用门静脉隔膜切除 1例 ,自体血管搭桥 1例 ,肠腔分流 3例 ,脾切除、脾肾分流 7例。结果 :1例死亡 ,11例治愈 ,随访 6月～ 70个月 ,再次出血 1例。结论 :肝前性门静脉高压症与其他类型的门静脉高压症在临床表现方面有许多不同之处。
Objective:To investigate the clinic features and the diagnosis and treatment of prehepatic portal hypertension.Methods:3 cases of non cirrhotic portal fibrosis and 9 cases of extrahepatic portal venous obstruction were conformed by angiography and operation.Besides the similar clinical manifestations as prehepatic portal hypertension,the patients presented a younger age and a normal liver function.Membranotomy was performed in 1 case,mesoportal shunt by autograft of blood vessel in 1 case,mesocaval shunt in 3 cases,and splenectomy and splenorenal shunt in 7 cases.Results:1 case died.11 cases were cured the time of follow-up was 6～70 months.Rehaemorrhase occurred in 1 case.Conclusion:There are many differences in clinical manifestations between prehepatic portal Hypertension and cirrhotic portal hypertension.Shunt may be the basic operation.
