光镜下血尿中红细胞形态观察计数的临床意义 被引量:1

CLINICAL VALUE OF MORPHOLOGICAL OBSERVATION AND COUNTING OF PBC IN BLOODY URINE UNDER OPTICAL MICROSCOPE

摘要 目的 探讨光学显微镜下血尿中畸形红细胞计数对肾小球性血尿与非肾小球性血尿定位诊断的意义。方法 用光学显微镜观察了220例血尿标本,根据尿中出现的红细胞的形态变化将其分为多形态畸形红细胞、单一形态畸形红细胞、正常形态红细胞3类,以出现多形态畸形红细胞作为判断肾小球性血尿的标准。结果 220例血尿标本光镜下红细胞形态在肾小球性血尿中,以多形态畸形改变的红细胞增多为主(87.6%),而在非肾小球性血尿中则是以正常形态红细胞为主(93.9%),未见有多形态畸形红细胞者。以出现多形态畸形红细胞作为判断肾小球性血尿,特异性为100.0%,敏感性为87.6%。结论 光镜下直接观察、计数血尿中红细胞的形态、数量,方法简单,无需特殊设备,并且对血尿的定位诊断敏感性高,特异性强,是一项简单、可行的判断血尿红细胞来源的无创伤性的初筛手段。 Objective To explore the clinical value of counting abnormal red blood cell(RBC)in bloody urine under optical microscope in localizing and diagnosing glomerular bloody urine. Methods The RBCs in 220 cases of hematuria were observed under optical microscope and classified into pleomorphic abnormal RBCs, single morphological abnormal RBCs and normal morphological RBCs. The specimens which were found to have pleomorphic abnormal RBCs were diagnosed as glomerular hematuria. Results The increase of RBCs of pleomorphic abnormal changes was found in most specimens of glomerular hematuria(87. 6%). Most RBCs had normal form (93.9%) in the specimens of non - glomerular hematuria and no pleomorphic abnormal RBCs were found in these specimens. When pleomorphic abnormal RBCS were used to decide whether the hematuria was glomerular or not,the specificity and sensitivity in location were 100.0% and 87.6% respectively. Conclusion Direct observation and counting of RBCs in hematuria have proved to have high sensitivity and specificity in location and can act as a primary screening measure in judging the origin of RBCs in bloody urine.
