摘要
目的研究CT三维重建技术(3DCT)在先天性高肩胛症(CES)中的应用,为全面了解CES的病理变化提供帮助。方法16例单侧CES,均采用螺旋CT扫描,用表面遮盖显示法(SSD又称3D)进行三维重建。3DCT图像分别从前、后、上、下、斜位观察,并测量肩胛骨上移的距离(EDSA)、锁骨倾斜度(CIA)、肩胛上角弯曲度(CSA)及肩胛骨旋转度(SRA)。结果16例肩胛骨均有不同程度的上移、体积变小、旋转、肩胛上角弯曲、锁骨向上倾斜。其中,发现肩椎骨5例、脊柱后方异常骨棘3例、颈肋2例,伴脊柱侧弯4例。患侧与健侧比较,肩胛骨平均上移3.8±1.6cm,锁骨向上倾斜12.6°±3.9°,肩胛骨下极向内旋转21.5°±4.2°,肩胛上角向前、下弯曲29.8°±12.4°。分度不同,肩胛骨的改变程度不同。结论3DCT能够清晰地显示CES的骨性病理改变,通过旋转可以在任意角度观察,并能定量测量具体反映肩胛骨的形态,有利于术前更全面了解其病理改变。
Objective To study on the application of three-dimensional computerized tomogr aphic(3DCT)reconstruction on congenital elevated scapula(CES).Methods 16 cases of CES were scanned with serial spiral CT and the images were three-dimen sionally reconstructed with shaded surface display (SSD)technology.Then the S SD-images were observed from anterior,posterior,inferior,superior and inclin e plane.The elevating distance of superior angle(EDSA),clavicle incline angl e (CIA),curvature of superior angle (CSA)and scapular rotation angle (SRA )were measured at the same time.Results These16scapulae were ascended,dimi nished and rotated in different degree.And the superior angle of scapula was b ended anteriorly.While the clavicle was inclined superiorly.Among these cases ,five scapulo-vertebral bones,three abnormal bone thorns and two neck-ribs were found out.Four cases were accompanied with scoliosis.In these cases,the mean values of EDSA,CIA,CSA and SRA were3.8±1.6cm,12.6°±3.9 °,29.8°±12.4°and21.5°±4.2°,respectively.Changes of scap ula in different cases of Cavendish catalog were varied.Conclusions The patho logical changes of congenital elevated scapula could be displayed clearly by3DC T.The SSD-images could be observed at any direction by rotating in three-dime nsional space,and the osteal pathom orphological changes of C E S m ight clearly display by quntitativve detection w ith SSD im aing,so it has beneficial to understond m ore com pletely the C E S in the preoperation.
